6.D. - Page 6 of 6 <br />Exhibit A <br />SCOPE OF SERVICES AND FEE <br />Consultant will perform supplemental design, construction support, and project close-out <br />services listed below related to the Rule 20 Middlefield Road Phase 1 Underground Utility <br />District Project, as directed by the City. <br />Task Description <br />Budget <br />• Revise plans to reflect multiple design changes to the joint utility <br />trench configuration and substructure as required by utility <br />companies <br />• Revise plans based on construction requirements and provisions <br />imposed by utility companies during construction <br />• Revise plans to incorporate additional Comcast conduits, vaults, and <br />boxes omitted from original Comcast Intent Drawings <br />• Revise Form B (descriptive list of work items and unit costs, including <br />$69,966 <br />a breakdown of proportionate share cost amounts for each <br />participating utility company) to reflect the multiple design changes <br />described above <br />• Prepare Record Drawings incorporating all changes/revisions from <br />the original, approved, contract documents <br />• Prepare Final Form B at completion of project <br />• Assist with preparation of final Form B invoicing to utility companies <br />REV: 04-17-2020 PR <br />ATTY/AGR.Amend No. 3/Bellecci & Associates (Page 3 of 3) <br />112 <br />
