<br />Exhibit Ai <br />Program/Project Description <br />City of Redwood City/Fair Oaks Community Center <br />Motel Voucher Early Entry Shelter for Singles Pilot Program <br /> <br />In consideration of payments provided in this Agreement, Contractor shall, under the general <br />direction of the Director of the Human Services Agency, or his authorized representatives, provide <br />services as described below: <br /> <br />1. CLIENTS TO BE SERVED: Single homeless individuals seeking shelter services who are <br />unable to be served due to lack of shelter space at one of the existing San Mateo County shelter <br />programs. <br /> <br />2. CLIENT SERVICES: Contractor will provide emergency housing to 50-75 unduplicated <br />individuals for the term of this Agreement. <br />The Contractor will perform the following services, supporting activities and related information from <br />January 1, 2008 through June 30, 2008; <br /> <br />A. Contractor will identify targeted homeless individuals who are coming to the center in need of <br />shelter services, have demonstrated their commitment to improve their self sufficiency level, <br />and are willing and eligible to enter shelter anywhere in San Mateo County. <br />B. Provide needs assessments. <br />C. Provide motel voucher services while the clients are waiting to receive shelter care services <br />with the allocated motel average rate of $60.00 per day as long as clients are client with <br />program guidelines. <br />D. Provide initial case management using Fair Oaks existing staff. <br />E. Facilitate client's entry to Maple Street Shelter, Safe Harbor Shelter, or a more appropriate <br />facility. <br />F. Provide supportive services and connect the client to main stream services. <br /> <br />3. EVALUATION: Contractor will collect and report the following data to the Director of <br />Prevention and Early Intervention by the end of the program. <br /> <br />Housing Outcomes: <br /> <br />A. Number of unduplicated individuals requesting shelter who enter shelter program to increase <br />by 75% over pre-EESP figures. <br />B. Percentage of unduplicated individuals requesting shelter who enter shelter program to <br />increase by at least 10% over pre-EESP figures. <br />C. Length of stay at the shelter. <br /> <br />Self Sufficiency Outcomes: <br /> <br />A. Number and percentage of clients who have been referred to HSA for benefits assessment. <br />B. Number and percentage of clients who have moved from shelter into permanent or more <br />structured housing. <br /> <br />1 <br />
