<br />You are hereby directed to make the herein described changes from the plans and specifications or do the following described
<br />work not included in the plans and specifications of this contract. All new work herein described shall be done in accordance
<br />with the applicable provisions of the plans and specifications, except as specifically modified by this contract change order.
<br />NOTE: This change order is not effective until approved by the City. This change order includes all costs and time required to
<br />complete the additional work described herein and the contractor shall be entitled to no other compensation.
<br />DESCRIPTION OF CHANGE COST
<br />At the intersection of Jefferson Avenue and Marshall Street:
<br />With the exception of asphalt concrete, the contractor shall provide all labor,
<br />equipment, material, and incidentals to reconstruct the intersection of Marshall
<br />Street and Jefferson Avenue, complete in place for a lump sum cost of
<br />$222,223.14. The contractor's scope of work explicitly includes but is not limited
<br />to the following:
<br />a. The contractor shall be responsible for all traffic control associated with
<br />this work
<br />b. The contractor shall be responsible for lowering the lids/rims of all utilities
<br />within the work area to grades shown on the project plans
<br />c. The contractor shall be responsible for sawcutting, demolishing, off
<br />hauling, and dumping the existing pavement as needed to meet the
<br />grades specified by on Sheet 6 of the project plans while providing a
<br />minimum 9" thick asphalt concrete section within the intersection
<br />d. The City will be purchasing the asphalt concrete from Granite Rock
<br />e. The contractor shall be responsible for installing a 9" thick deeplift of
<br />asphalt concrete within the limits of work shown on the project plans,
<br />complete in place
<br />f. The contractor shall be responsible for completing all remaining traffic
<br />signal modification work as shown on the project plans and project
<br />specification
<br />g. The contractor shall be responsible for completing all signing and striping
<br />work as shown on the project plans and project specification
<br />h. The field modified striping changes
<br />$222,223.14
