6.J. - Page 25 of 86 <br />Attachment B <br />Bidder's Initials <br />The term shall be from the effective date of the contract until services have been rendered <br />completely. The City shall notify the Contractor in writing of the intent to extend the contract. <br />B. Invoicing <br />Contractor will submit monthly invoices to: Public Works Services Department, Attn: Daniel <br />Barros, 1400 Broadway, Redwood City, CA 94063. A sample invoice must be included in the <br />submitted bid. <br />C. Payment <br />If a contract is awarded, the method of payment to the Successful Bidder shall be on an <br />hourly, per task basis, with a not to exceed amount as set by the Successful Bidder in the bid <br />or as negotiated between the Successful Bidder and the City. Payment will be made within <br />thirty (30) days of receipt of an accurate invoice. Payment will be made no more frequently <br />than monthly. <br />D. DIR Registered Contractor <br />The Contractor must be registered with the DIR pursuant to Labor Code §§ 1772.5 and 1771.1, and <br />provide its DIR contractor number in Attachment A— Bid Pricing Form. <br />E. Redwood City Business License <br />The Successful Bidder must either possess a current, valid Redwood City business license or <br />have submitted a Redwood City business license application and fee at the time of contract <br />award. Business license information (application, ordinance, fees) can be found by navigating <br />to the City of Redwood City homepage (www.redwoodcity.org) and selecting "Business," <br />"Starting a Business," "Business Licensing." <br />F. Bid Cost <br />The Contractor must complete, sign and submit Attachment A — Bid Pricing Form with <br />Contractor's bid. All pricing must be inclusive, and include all labor, material, and equipment <br />necessary for all tasks listed in this Scope of Services. Provision of this information assists the <br />City in determining whether the Bidder understands the project, whether the costs are fair <br />and reasonable in light of the services to be provided, and provides City staff with tools to <br />negotiate the final cost. Bidder may submit a detailed breakdown of the costs of their total <br />bid as a supplemental document but total bid price must be summarized and submitted on <br />Attachment A — Bid Pricing Form <br />Labor Rates shall be bid at union prevailing wage rates and calculated using straight time <br />hours. <br />Page 8 of 27 <br />443 <br />
