<br />EXHIBIT "A" <br /> <br />The Licensor's duties pursuant to this Agreement are as follows: <br /> <br />1. The use of the Premises shall be rendered to the Licensee free of charge in <br />exchange for services rendered to citizens of the City of Redwood City. The <br />Ombudsman Services of San Mateo County, Inc. shall submit an annual <br />report describing how Redwood City residents were served by this program. <br /> <br />2. The Licensor shall provide the Premises located at Veterans Memorial Senior <br />Center, 1455 Madison Avenue, City of Redwood City, and its neighboring <br />Wellness Center located at 711 Nevada Street, City of Redwood City, <br />consisting of: a) Reception Office; b) Rooms 28, 29 & 30 for five office <br />spaces M-S 7-8; c) Sunset Room 4th Thursday of the month 9:30am-12:30, <br />and d) Room 20 2nd Friday of the month 9am-12n. Any other space use will <br />be scheduled according to availability. <br /> <br />3. Work closely with the Ombudsman Services of San Mateo County, Inc. by <br />participating in regular meetings to establish effective and ongoing <br />communication. Staff from both the Veterans Memorial Senior Center and <br />the Ombudsman Services of San Mateo County, Inc. will discuss and <br />contribute to decisions made by the Manager of the Veterans Memorial <br />Senior Center and the Ombudsman Director relating to quality and/or levels <br />of services to be provided. <br /> <br />4. The Manager at the Veterans Memorial Senior Center and the City's Park's, <br />Recreation and Community Services staff shall partner with the Ombudsman <br />Director and the Ombudsman Services of San Mateo County, Inc. staff to <br />recruit and train Information and Referral Program volunteers. <br /> <br />5. The Manager of Veterans Memorial Senior Center and the Director of <br />Ombudsman Services of San Mateo County, Inc. will evaluate the Information <br />and Referral Program on a 3 month, 6 month and 12 month calendar for the <br />first year to be renegotiated at the annual evaluation. <br /> <br />6. The Manager at the Veterans Memorial Senior Center and staff shall <br />continue to coordinate services and new partnership opportunities between <br />Ombudsman Services of San Mateo County, Inc. and the City of Redwood <br />City's Parks, Recreation and Community Services Department. <br /> <br />ATTY/AGR/2009.009 <br />021909 <br /> <br />7 <br />
