<br />ATTACHMENT 6 <br /> <br />Facility Permits <br /> <br />The following Facility permits are the responsibility of the SBWMA to obtain and <br />renew, any permit not listed below is the responsibility of the Contractor to obtain <br />and/or renew: <br /> <br />o Solid Waste Facility Permit with the California Integrated Waste <br />Management Board (CIWMB) (SWIS 41-AA-0016) <br />o City of San Carlos Land Use Permit <br />o Bay Area Air Quality Management District (BAAQMD) (#11530) <br />o State Water Resources Control Board Storm Water Discharge Permit <br />(#241100897) <br />o Facility Fire Permits (City of Redwood City and County of San Mateo) <br /> <br />The following permits are examples of permits that are the responsibility of the <br />Contractor to obtain and renew*: <br /> <br />o Spray booth permit <br />oUST and AST permits <br />o SQG Permit <br />o Compress~d Air Tank permits <br />o Hazardous Waste Generator Materials Management Permit (San Mateo <br />County EH&S) <br />o Department of Motor Vehicles Motor Carrier Permit <br />o Mobil Crane Permit <br />oBi-annual T erminallnspection Permit <br />o Waste Tire Hauler Permit <br /> <br />* The above list is not intended to be comprehensive. Additional permits that are <br />identified during the transition of facility operations in 2011 will be added to the above <br />list.
