6.C. - Page 54 of 147 <br />Contractor Safety Checklist Cargill, Inc. <br />CONTRACTOR EMPLOYEE INFO <br />Date: <br />Contractor Company Name: <br />Contract Employee Name: _ <br />PURPOSE <br />These guidelines are intended as general guidance for contractor and subcontractor activities concerning <br />safety performance while performing work on Cargill Salt property. Contractors, subcontractors, and <br />their employees and agents (hereafter referred to collectively as "contractors") will be required to meet <br />all applicable local, state, and federal laws and regulations which will take precedence over these <br />guidelines in event of a conflict, unless the Cargill guideline is more restrictive. <br />The contractor employee shall place his/her initials on each line item, as they are reviewed. A copy of <br />the entire document shall be offered to the contractor for reference of Cargill guidelines. <br />CONTRACTOR RESPONSIBILITIES <br />1. To ensure that contractor/subcontractor employees and agents on Cargill property are familiar with <br />and follow all guidelines set forth in this document. Each contractor personnel is required to review <br />and understand this checklist their first time on site and annually thereafter. <br />2. To ensure that all employees and agents of contractors are physically able and properly trained to <br />perform their duties. <br />3. The contractor supervisor's responsibility to know these rules and ensure they are followed by <br />his/her employees. <br />4. The contractor is responsible for the safety and conduct of their employees. <br />5. To report all incidents and hazardous conditions to Cargill immediately. All incidents are to be <br />investigated by the contractor within 24 hours and a copy of the investigation report to be given to <br />Cargill within 48 hours. <br />6. To maintain and furnish to each of their employees all personal protective equipment required. <br />7. Stop and consult with Cargill if the nature of the job deviates from original scope, before <br />proceeding. <br />Non-compliance of any of these rules may result in the removal from company premises. <br />CONTRACTOR SAFETY RULES AND PROCEDURES <br />1. Before beginning any work, each day a pre job hazard assessment (PJHA) and discussion is <br />recommended with all personnel performing the work. <br />2. All contractors must wear hard hats, safety shoes that meet ASTM F 2412-05 and 2413-05 <br />standards, gloves appropriate to the work, safety glasses that meet ANSI Z87.1-2003, long <br />pants and sleeved shirts whenever they are on Cargill property. Hearing protection must be <br />worn in posted areas. Without the proper PPE contractors will not be allowed to work. Knee <br />and body pad use is encouraged to prevent contact stress injuries. Face shields in addition to <br />safety glasses or goggles are required for grinding, blowing with compressed air, and any <br />operation where there is a risk of being struck in the face.
