EXHIBIT "A" <br />SCOPE OF SERVICES AND FEE <br />EXHIBIT "A" <br />Scope of Services <br />The scope of services described herein is the minimum necessary to meet the City's objectives. <br />The consultant is expected to expand on the scope by incorporating their expertise and proposed <br />method of approach. <br />The scope of work for the project shall include the following: <br />1. Furnish all labor, materials, and equipment necessary to clean pipeline, inspect manholes and <br />pipelines and provide a closed-circuit television inspection in digital format of the sewer <br />pipelines listed in the table. The word "clean" is defined as the removal of all accumulations <br />including sludge, dirt, sand, rocks, grease, roots, and other solid or semisolid material in the <br />pipe and manhole. <br />2. The CONSULTANT shall be solely responsible for traffic control. As a minimum, placement <br />of adequate pre -warning and detour signs to control vehicular and pedestrian traffic shall be <br />required. Certain arterial or commercial district streets may require night work. Additional <br />compensation for night work will not be paid by the City. <br />3. If entry to confined spaces is necessary, the CONSULTANT shall follow all appropriate local, <br />state, and federal laws pertaining to confined space entries. Sanitary sewer manholes are <br />considered confined spaces. Specific attention is directed also to OSHA safety rules, <br />regulations and precautions to be taken by the Contractor before entering sanitary sewer <br />manholes, and other sanitation structures with respect to physical and chemical hazards that <br />may be present. <br />4. The CONSULTANT shall protect the manholes to withstand forces generated by equipment, <br />water, and air pressure. The CONSULTANT shall clean the sewer pipe of any obstruction <br />and debris including roots in accordance with the "Standard Specifications for Public Works <br />Construction (Green book)", latest edition. <br />5. The CONSULTANT shall perform a minimum of two (2) passes using the high -velocity <br />hydraulic cleaning method. The contractor shall clean the line until the line is ready for closed- <br />circuit television inspection that is acceptable to City. No additional compensation will be made <br />to the CONSULTANT in performing three or more passes in cleaning the sewer line. All pipe <br />cleaning cost method and duration shall be included in the cost proposal irrespective of the <br />type and size of the existing sewer lines. If a video and/or the capture images in the database <br />do not clearly show the pipe conditions due to inadequate clean-up or are of such poor quality <br />that the Engineer is unable to evaluate the condition of the sewer or locate sewer service <br />connections, the CONSULTANT shall re -televise that section of the pipe at CONSULTANT <br />expense. <br />6. All water used in conjunction with the project improvements shall be metered and paid for by <br />the CONSULTANT: <br />a. CONSULTANT shall pay an $800 deposit to Cash Collections at City Hall, 1017 <br />Middlefield Road for the construction meter.
