EXHIBIT "A" SCOPE OF SERVICES AND FEE EXHIBIT "A" Scope of Services The scope of services described herein is the minimum necessary to meet the City's objectives. The consultant is expected to expand on the scope by incorporating their expertise and proposed method of approach. The scope of work for the project shall include the following: 1. Furnish all labor, materials, and equipment necessary to clean pipeline, inspect manholes and pipelines and provide a closed-circuit television inspection in digital format of the sewer pipelines listed in the table. The word "clean" is defined as the removal of all accumulations including sludge, dirt, sand, rocks, grease, roots, and other solid or semisolid material in the pipe and manhole. 2. The CONSULTANT shall be solely responsible for traffic control. As a minimum, placement of adequate pre -warning and detour signs to control vehicular and pedestrian traffic shall be required. Certain arterial or commercial district streets may require night work. Additional compensation for night work will not be paid by the City. 3. If entry to confined spaces is necessary, the CONSULTANT shall follow all appropriate local, state, and federal laws pertaining to confined space entries. Sanitary sewer manholes are considered confined spaces. Specific attention is directed also to OSHA safety rules, regulations and precautions to be taken by the Contractor before entering sanitary sewer manholes, and other sanitation structures with respect to physical and chemical hazards that may be present. 4. The CONSULTANT shall protect the manholes to withstand forces generated by equipment, water, and air pressure. The CONSULTANT shall clean the sewer pipe of any obstruction and debris including roots in accordance with the "Standard Specifications for Public Works Construction (Green book)", latest edition. 5. The CONSULTANT shall perform a minimum of two (2) passes using the high -velocity hydraulic cleaning method. The contractor shall clean the line until the line is ready for closed- circuit television inspection that is acceptable to City. No additional compensation will be made to the CONSULTANT in performing three or more passes in cleaning the sewer line. All pipe cleaning cost method and duration shall be included in the cost proposal irrespective of the type and size of the existing sewer lines. If a video and/or the capture images in the database do not clearly show the pipe conditions due to inadequate clean-up or are of such poor quality that the Engineer is unable to evaluate the condition of the sewer or locate sewer service connections, the CONSULTANT shall re -televise that section of the pipe at CONSULTANT expense. 6. All water used in conjunction with the project improvements shall be metered and paid for by the CONSULTANT: a. CONSULTANT shall pay an $800 deposit to Cash Collections at City Hall, 1017 Middlefield Road for the construction meter.
