When possible, include access to perishable food as well, including <br />fresh produce. <br />Utilize Second Harvest Silicon Valley and/or other resources to obtain food. <br />Provide referral list of food resources where clients can receive additional food resources, <br />such as meal programs and grocery programs. <br />Assist clients with applying for and accessing food via Second Harvest Silicon Valley and <br />other community food programs. <br />Financial Assistance <br />Provide rent assistance, mortgage assistance, security deposits, utilities assistance, car <br />repair, and other emergency financial needs to eligible households using all available <br />funding sources such as Measure K Homeless Prevention Assistance Program, Season of <br />Sharing, Community Services Block Grant (CSBG), and other available funding, following <br />the guidelines and eligibility criteria established for each funding source. Provide <br />application information to potential applicants (via information on Contractor’s website and <br />other communication processes), process applications received either online or in paper, <br />follow-up with applicants regarding any missing or unclear information in their application, <br />and review documents submitted and determine eligibility. Refer clients to other available <br />resources such as Housing Industry Fund (HIF) and St. Vincent de Paul. <br />Refer clients to utility assistance programs and liaise with the appropriate entities such as <br />Central Coast Energy Services to submit the application and assist clients with completing <br />the application process. <br />Material Goods <br />Assist clients with accessing resources for clothing, diapers, and other emergency material <br />needs. <br />Transportation <br />Provide referrals to transportation programs including Redi-Wheels. <br />Arrange for emergency transportation and assistance with the cost of fares. <br />Participate in the Lifeline Transportation Program for taxi vouchers, bus passes and bus <br />tickets, when available. See Exhibits B1 and C1 for more information. <br />Tax Credits <br />Assist tax filing and access to tax credits via referral. <br />Promote tax preparation services to clients/the community to encourage access to tax <br />credit programs. <br />Inclement Weather <br />Participate in the Inclement Weather Program. <br />Outreach <br />Provide information to community members regarding how to access safety net and <br />homeless services by attending community meetings/events, collaborating with city <br />agencies, community stakeholders and other outreach activities. <br />Information & Referral <br />Provide information and referrals to meet the needs of clients, including the services above, <br />as well as adult education, employment, physical and behavioral health needs, and other <br />needs. <br />ATTY/AGR.2021.130/County of San Mateo (Page 11 of 31)
