Template Version August 26, 2016 City of Redwood City - 75100-22-D004 <br /> <br />REV: 06-04-2021 RL <br />Exhibit B1 - Lifeline Transportation Program Services Payments and Rates <br />In consideration of the services set forth in Exhibit A1, and the terms and conditions of the Agreement, <br />County shall pay contractor based on the following terms: <br />I. County shall pay Contractor quarterly upon receipt and approval of quarterly itemized invoices <br />and reports as follows: <br />A. Contractor shall submit a quarterly invoice, with backup documentation for Services <br />shown in Exhibit A1, within 20 days after the end of the quarter. All invoices and <br />supporting documentation shall be submitted electronically to Ana Morales at <br />AMorales1@smcgov.org or the designated HSA contact. See Exhibit A1 for more <br />information regarding Lifeline Transportation Program (LTP) requirements. <br />Back-up documentation will include: <br />o Receipts for all transportation assistance purchases in support of the invoice amount. <br />o Quarterly Summary Report <br />o Quarterly LTP Control Log <br />II. Availability of Funds: County may terminate this Agreement in whole or a portion of services <br />based upon availability of funds by providing a thirty (30) day written notice to Contractor. <br />III. Performance: County reserves the right to withhold payment if County determines that the <br />quantity or quality of the work performed is unacceptable. If County should find that the quantity <br />or quality of work is unacceptable, County shall notify Contractor in writing with a detailed <br />statement and plan to correct performance. Contractor shall respond to County within fifteen <br />(15) days of receipt of statement with a plan to confirm what steps will be taken to correct <br />performance. <br />IV. Additional Services: County may request additional related services under this agreement and <br />adjust program rates for the programs described within this agreement to accommodate the <br />addition of services as agreed upon by both parties as long as it does not exceed the total <br />agreement obligation and is not restricted by any grant or specific funding agreements. <br />V.
