REV: 07-20-21 SK <br />5) Rent and Move-In Assistance: <br />Program Administrator will provide payments to clients through a “flat subsidy” model <br />comprised of the below sliding scale for over 12 months: <br />up to $1,050 for deposit and $1,050/month for four individuals, or <br />up to $1,750 for deposit and $1,750/month for four families <br />This process will enable the quick resolution of clients’ immediate housing crises, and will <br />be tailored to each household’s ability to maintain housing once the subsidy ends. <br />Program Administrator will also make additional flexible funds (Flex Funds) available for <br />move in costs, deposits, utilities, and credit repair, depending on clients’ individual needs. <br />6) Unit Inspections: <br />All housing units must pass the HUD “Habitability” standard, through inspections by the <br />client’s CM or other program staff prior to the client’s move-in date. All Rapid Rehousing <br />Program staff will be trained by Program Administrator to complete the required HQS <br />Inspection Form. <br />7) Record Keeping: <br />Complete records of all clients, including admission, discharge, services utilized, <br />outcomes, and demographics will be entered and regularly updated by CMs. Program <br />Administrator will gather qualitative data through feedback from clients during program <br />activities, CM sessions, client satisfaction surveys, and anonymous suggestion boxes. <br />Program Administrator will submit complete, accurate, and timely quarterly reports, <br />Performance Measures <br />The chart below outlines the performance measures and targets expected to be met by <br />LifeMoves: <br />Performance Measure Target <br />1 Percent of the households <br />enrolled in the Temporary <br />RV Safe Parking Program <br />who move into permanent <br />housing within 45 days of <br />referral to the RV Rapid <br />Rehousing <br />Program <br />65% <br />2 Percent of households <br />enrolled exiting to <br />permanent housing <br />90% <br />3 Percentage of households <br />served who maintain <br />80% <br />ATTY/AGR.2021.192/Lifemoves (RAPID REHOUSING PROGRAM ADMINISTRATOR) (Page 13 of 16)
