REV: 09-01-2021 RL <br />EXHIBIT “B” <br />DECLARATION REGARDING WORKERS’ COMPENSATION AND EMPLOYER’S LIABILITY <br />INSURANCE <br />Contractor certifies that it has only three employees, that such employees are equal <br />owners of the firm and have elected not to be covered by workers’ compensation <br />insurance in accordance with an exception to the requirement of workers’ compensation <br />insurance coverage as provided for under Section 3351 (f) of the California Labor Code. <br />Contractor agrees to purchase workers’ compensation insurance if it retains any <br />additional employees during the term of this agreement.
