REV: 10-18-2021 RL <br />EXHIBIT A <br />Project Description <br />This document is the Project Description contemplated by the Memorandum of Understanding, <br />dated July 1, 2021 between Redwood City Together (the “Consultant” or “RCT”) and Friends for <br />Youth (“FFY”). <br />I. Basic Project Data: <br />Project Background: The WHY Initiative was funded in August of 2018 through the <br />Sand Hill Foundation’s Wellness Partnership to provide more <br />integrated preventative mental health services for youth and <br />young adults. WHY provides holistic prevention and early <br />intervention services to youth and families in North Fair Oaks, <br />Redwood City, and surrounding communities. The collaborative <br />includes three youth-serving agencies—Friends for Youth, <br />Redwood City PAL, and Siena Youth Center—and three <br />resource providers— Peninsula Conflict Resolution Center, One <br />Life Counseling Center, and Star Vista. <br />The mission of the WHY Initiative is to improve youth mental <br />health by providing accessible wellness programs, education, and <br />resources to youth and those who support them. <br />The WHY Initiative’s shared goals are to: <br />1. Improve access to preventative mental health and <br />wellness services through increased coordination and <br />improvements to policy and practice. <br />2. Collectively serve 620 youth, 50 providers, and 200 <br />parents. <br />3. Improve the quality of participants’ social emotional <br />health and support network. <br />4. Improve community awareness around mental health <br />and wellness and how to access resources. <br />5. Strengthen partner relationships, collaboration, and <br />sustainability efforts. <br />Project Purpose FFY, the lead agency for the WHY Initiative, will partner with <br />Redwood City Together as a consultant beginning October 1, <br />2021. Redwood City Together will work to enhance the WHY <br />Initiative’s collaborative services, improve systems-level <br />coordination, and advance the sustainability of strategic goals. <br />Redwood City Together will support the collaborative activities <br />and sustainability of the partnership. <br />Project Start Date: October 1, 2021 <br />Project End Date: June 30, 2022 <br />Primary Contact at FFY: Cecilia Chu <br />cecilia@friendsforyouth.org <br />ATTY/AGR.2021.264/Friends for Youth (Page 6 of 8)
