REV: 10-18-2021 RL <br />goals from the Sand Hill and Dignity Health grant proposals <br />(Deadline: July 23, 2021). <br />Facilitate five agreed upon community stakeholder interviews of those <br />outside of the WHY Initiative to gain qualitative feedback on the <br />following three areas: <br />1) The primary youth and family mental health & wellness needs/ <br />gaps in services in the stakeholder’s area of expertise (i.e., school <br />district, county health). <br />2) Their primary prevention and early intervention goals and <br />priorities. <br />3) Their vision for how the WHY Initiative partners can deepen <br />collaborative services for marginalized populations <br />Potential Stakeholders for Interviews: San Mateo County CCCS, <br />Sequoia Healthcare District, Redwood City School District, <br />Sequoia Union High School District, Cañada College, Edgewood <br />TAY Drop-In Center, BHRS, Star Vista School-Based Counseling <br />Services and/or other agreed-upon organizations. <br />Deliverables: <br />High-level 1 page summary of primary needs and priorities <br />(Deadline: July 23, 2021) <br />30 minute presentation of data crosswalk and 1 page <br />summary at the first strategic planning session <br />c. Conduct four community roundtable(s) with a minimum of eight attendees each to <br />gain a diverse sample size of youth, parent, and provider feedback on WHY <br />vision and priorities (i.e, two in September for strategic planning; total of 12 <br />hours by June 30, 2022). Deliverable: High-level 1 page summary of community <br />feedback after each roundtable. <br />2. Plan and facilitate working group convenings to advance strategic goals over the remainder <br />of the year including service implementation and evaluation. Convening frequency and <br />structure will be determined during strategic planning. (4-5 hours per month; 50 hours by <br />June 30, 2022). <br />3. Provide consultation and resources on community needs, best practices, advocacy efforts, <br />key stakeholders, and funding opportunities. Connect partners to stakeholders who can <br />support sustainability including at least one new collaborative grant opportunity (20 hours <br />by June 30, 2022). <br />4. Assist with the promotion of WHY Initiative programs and events including outreach to <br />youth, families, and cross-sector partners (20 hours by June 30, 2022). <br />5. Maintain and submit accurate reporting of services, hours, and use of funds by January 1, <br />2022 and June 30, 2022 to FFY. <br />IV. Disbursement of Contract Funds: <br />FFY will disburse the first half of contract funds of $5,000 to RCT upon receipt of this MOU <br />signed by the Executive Directors of FFY and RCT.
