<br />6.1C <br />Page 13 <br /> <br />EXHIBIT A <br /> <br />CITY OF REDWOOD CITY <br />FAMILY SERVICE AGENCY OF SAN MATEO COUNTY <br />FISCAL YEAR 2009/2010 <br /> <br />CONTRACT OBJECTIVES <br /> <br />Quantifiable Goals to be Provided With <br />Re uested Funds <br />By June 30, 2010, 360 seniors will not go <br />hungry by having at least one meal. either <br />breakfast or lunch. fulfilling one third of <br />their re uired dieta nutritional needs. <br />By June 30, 2010, 20 seniors will <br />overcome barriers to limited mobility to <br />access center services and for other <br />needs through coordination of Redi- <br />Wheels Paratransit and the TRIP <br />fO ram. <br />By June 30, 2010. 77 seniors will be <br />linked to services they need to maintain <br />their stability, including housing programs, <br />transportation, legal aid and health <br />roblems. <br />By June 30, 2010, 32 unduplicated <br />seniors will participate in blood pressure <br />screenings and preliminary health checks <br />through visits from the Sequoia Hospital <br />registered nurse while the Case Manager <br />provides translation and medical follow <br />u . <br />By June 30, 2010, we will have a 5% <br />increase in new Redwood City <br />participants through outreach and <br />marketin .- <br /> <br /> <br /> <br />Family Service staff assesses all clients for <br />eligibility for Redi-Wheels, and assist with <br />application process, scheduling and distribution <br />of tickets. <br /> <br />The Case Manager develops a service plan for <br />all clients and provides follow up to ensure <br />issue is resolved. <br /> <br />Seniors are required to sign in at the blood <br />pressure screenings. <br /> <br />New participants for nutrition, case <br />management and transportation are required to <br />complete an intake form (100x). <br />I and R client cases will be documented. <br /> <br /># of Unduplicated RWC Residents/Households to be Assisted (Only programs serving <br />individuals may report by individuals. All others should report by households) <br /> <br />200 Redwood City residents will be served by the programs at Fair Oaks. <br />
