Contract Template <,000 <br />May 2021 <br /> Page 11 <br />REV: 06-16-22 SK <br />Exhibit A – Core Services <br />In consideration of the payments set forth in Exhibit B, Contractor shall provide the following services: <br />A. Designated Service Area <br />Contractor will provide the following services in North Fair Oaks, Redwood City, Portola Valley, <br />Redwood Shores, Atherton, and Woodside. <br />B. Population <br />1. Contractor will serve low-income San Mateo County (County) adults, youth, and families who <br />reside in the Contractor’s designated service area (indicated above) of San Mateo County. <br />These residents will be experiencing various challenges including maintaining housing, <br />potential evictions, food insecurity, and are at-risk of or experiencing homelessness. Contractor <br />will provide these residents with emergency and safety net services to meet immediate and <br />basic needs. Services include food, housing, homeless prevention services, emergency <br />financial support including rent and/or utility assistance, shelter, clothing, and/or transportation. <br />2. For San Mateo County residents outside of the Contractor’s designated service area, <br />Contractor will provide immediate and basic needs and then refer the resident/s to their <br />designated Core Service Agency. <br />3. For San Mateo County residents experiencing homelessness, Contractor will provide services <br />regardless of the resident’s service area. <br />4. For non-San Mateo County residents seeking services, Contractor will provide information <br />regarding how the resident/s can access services in their home community. <br />C. Services to be Provided <br />Contractor/Core Service Agency will: <br />1. Perform a client-centered, strengths-based assessment for each household or individual, <br />determine client’s needs, and provide services based upon the unique needs of the <br />household/individual. <br />2. Provide services in a culturally sensitive manner. <br />3. Provide services in languages matching the demographics of the population to be served and <br />to outreach to potential clients. At a minimum, services will be provided in English and Spanish. <br />Contractor will have processes in place to provide services to clients who speak languages <br />other than English and Spanish. <br />4. Provide staff training and supervision to ensure high quality, consistent services in compliance <br />with Contractor’s policies and procedures. <br />5. Provide public benefit information and assistance, Contractor will: <br />a) Maintain knowledge of public benefits and benefit program application processes. <br />b) Provide information about public benefits such as CalFresh, CalWORKs, General <br />Assistance, Cash Assistance Program for Immigrants (CAPI), unemployment benefits, <br />Women, Infants and Children (WIC), and health coverage programs, such as Covered <br />California, ACE (Access and Care for Everyone), and Medi-Cal. In addition, Contractor will <br />refer veterans to the County’s Veterans Service Office to answer questions or provide <br />support regarding eligible benefits specific to veterans. <br />c) Assist clients with applying for and maintaining (enrollment in) benefits. This includes <br />referring clients to appropriate public agencies and providing phone, online or paper <br />support to clients as necessary to apply for or maintain benefits. <br />6. Provide emergency food and connections to ongoing food resources. Contractor will: <br />a) Maintain a supply of emergency food on-site that can be distributed to clients with <br />immediate food needs. Contractor may utilize Second Harvest and/or other <br />resources to help obtain food for their on-site supply of emergency food.
