REV: 06-22-22 RL a) Gold Partner. i) A Community Partner that elects to be a Gold Partner pursuant to this paragraph 2(a) shall be entitled to priority advanced scheduling of distributions on a bi-weekly, monthly or quarterly basis (as determined by the needs of such Gold Partner), subject only to available inventory. ii) Gold Partners must enroll clients in the Diaper Bank program through HAMO’s “Client Enrollment Process”, which process shall include: (1) Client’s child must be under the age of three (36 months). (2) Client must complete and execute the Bay Area Diaper Bank Enrollment Form (attached hereto as Exhibit A and referred to hereinafter as the “Enrollment Form”). Client must complete and execute a separate Enrollment Form for each child that client seeks to enroll in the Diaper Bank program. (3) Gold Partner must submit the completed/fully executed Bay Area Diaper Bank Enrollment Form to HAMO via secure electronic methods (e.g., Docusign, DropBox Business, etc.) as soon as the client is accepted into the program. (4) Following receipt of the completed/fully executed Enrollment Form by HAMO, client shall be officially enrolled in the Diaper Bank program and shall be eligible to receive up to two (2) cases of diapers each month thereafter until the child reaches three years of age, subject to available inventory and the terms of the Enrollment Form. iii) Gold Partners agree to maintain timely and ongoing records of its diaper distributions to clients and shall furnish HAMO monthly reports detailing the total number of children and families served each month. iv) Gold Partner agrees to notify HAMO of relevant staff changes and shall work in good faith to ensure all new staff are trained to administer the diaper program within the guidelines of this Agreement and relevant program operating manuals. b) Silver Partner. i) Silver Partners shall not be required to enroll clients in the Diaper Bank program through HAMO’s Client Enrollment Process; provided however, Silver Partners are required to maintain timely and ongoing records of its diaper distributions to clients and shall furnish HAMO monthly reports detailing the total number of children and families served each month. ii) A Community Partner that elects to be a Silver Partner pursuant to this paragraph 2(b) shall be entitled to advanced scheduling of up to 12 distributions from the period of 7/1/2022 to 6/30/2023. iii) Silver Partner agrees to notify HAMO of relevant staff changes and shall work in good faith to ensure all new staff are trained to administer the diaper program within the guidelines of this Agreement and relevant program operating manuals.
