REV: 05-31-22 MI <br />EXHIBIT “A” <br />SCOPE OF SERVICES AND FEE <br />Contractor will perform rodent and cockroach control services for the City for the course of <br />the Agreement. <br />I.General Provisions <br />A.San Mateo County Mosquito and Vector Control District (SMCMVCD) <br />Inspection Fee <br />Contractor will pay an annual inspection fee of two dollars twenty-five cents <br />per bait station to SMCMVCD. The inspection fee is based upon the number <br />of bait stations and the frequency in which they are to be serviced. The <br />Contractor shall recover this fee only through the Inspection of Bait Station <br />fee and not as a separate charge. <br />B.Licenses <br />Contractor will possess a Qualified Applicator License, Category A, <br />(Residential, Industrial and Institutional), or Category K (Health Related), <br />issued by the State of California, Department of Pesticide Regulation, <br />Pesticide Enforcement Branch, or a Branch “2” Pest Control Operator’s <br />License issued by the Structural Pest Control Board of the State of <br />California, in force during the performance of all work under this Agreement. <br />Contractor will be Integrated Pest Management (IPM) certified or implement IPM <br />methods. <br />Contractor will inform the Public Works Director or designee if the license <br />expires, is revoked or otherwise becomes invalid. <br />C.Permits, Notices and Compliance with the Law <br />Contractor will be responsible for obtaining all permits necessary to work <br />within the public right-of-way. Contractor will give all notices required by law <br />and comply with all laws, ordinances, rules and regulations pertaining to the <br />conduct of the work. Contractor will adhere to the Integrated Pest <br />Management policy. Contractor will be liable for all violations of the law in <br />connection with work furnished by the Contractor. <br />If the Contractor observes that the specifications are at variance with any <br />law, ordinance, rule or regulation, Contractor will promptly notify the Public <br />Works Director or designee and SMCMVCD in writing. <br />In the Contractor performs any work knowing it to be contrary to such laws, <br />ordinances, rules and regulations, and without giving such notice to the <br />Public Works Director or designee and SMCMVCD, the Contractor will bear <br />all costs arising therefrom. <br />ATTY/AGR.2022.127/Vector Pest Control (Dewey Services, Inc.)
