REV: 11-10-22 RL <br />Coordinate and provide oversight of the Inter-Agency Redwood City Homeless <br />Outreach Team. <br />Incorporate peer outreach workers and individuals with lived experience as part of <br />the Homeless Outreach Team. <br />Incorporate sufficient bilingual English/ Spanish staff to meet need of primary <br />Spanish speaking individuals, <br />Create “by-name list” of unsheltered homeless individuals identified by the <br />Redwood City Homeless Outreach Team for outreach and engagement towards <br />housing and related services. <br />Work closely with San Mateo County Human Services Agency (HSA) and the City <br />on the annual One Day Homeless Count, and other ad hoc initiatives to collect <br />information regarding people experiencing homelessness. <br />Provide outreach and engagement to unsheltered homeless individuals and <br />Households2 in Redwood City: <br />Provide regular and ongoing outreach and engagement to assigned areas in <br />Redwood City and to “by-name” list of individuals; <br />Engage unsheltered homeless individuals and households to establish a pathway <br />to permanent housing and linkages to homeless system and mainstream services; <br />Provide rapid response to requests for homeless outreach staff to make contact and <br />provide support to unsheltered homeless individuals or households in Redwood <br />City; <br />Provide after hours and weekend coverage in Redwood City to address immediate <br />services or interventions for unsheltered homeless individuals and households that <br />may not be able to access mainstream services during business hours; <br />Conduct additional outreach and assist unsheltered homeless individuals and <br />households with accessing shelter, provide water and other supplies, and connect <br />individuals to other resources during inclement weather activations and other <br />emergencies and <br />Provide intensive coordinated outreach when an encampment clear out is planned. <br />2 “Household” means 2 individuals or more who identify as a single household <br />ATTY/AGR.2022.394/LifeMoves (Redwood City Coordinated Homeless Outreach) (Page 14 of 17)
