REV: 11-10-22 RL <br />Connect unsheltered homeless individuals and households in Redwood City to housing <br />and services: <br />Provide case management services to support unsheltered homeless individuals <br />and households’ transition to permanent housing; <br />Provide housing services to support transition to permanent housing; <br />Provide or connect unsheltered homeless individuals to mental health <br />support/services; and <br />Provide or connect unsheltered homeless individuals to substance disorder and <br />recovery support/services. <br />Provide resources for unsheltered homeless individuals and households in Redwood City: <br />Provide a Hotel Respite Program for unsheltered homeless individuals and/ or <br />households; <br />Provide supplies or other items to assist unsheltered homeless individuals or <br />households with meeting their basic needs and to build rapport (food, gift cards, <br />socks, glasses, etc.); <br />Provide transportation assistance to assist unsheltered homeless individuals or <br />households with connecting to services. <br />Document and track contacts and services provided: <br />Document and track all inquiries, responses, and case management plans in the <br />San Mateo County Clarity HMIS (Homeless Management Information System) in a <br />timely manner. <br />Utilize additional, complimentary tools that assist with documentation and tracking <br />Objectives: <br />During the term of the Agreement, Consultant will report on the following objectives: <br />Number and percentage of unsheltered homeless individuals who receive <br />outreach and engagement in Redwood City3 Goal: 90% by July 2023; <br />Number and percentage of unsheltered homeless households who receive <br />outreach and engagement in Redwood City Goal: 90% by July 2023; <br />Percentage of rapid response inquiries responded to within 24 hours of the <br />request being made – Goal: 90%; <br />Number of unduplicated unsheltered homeless individuals who receive case <br />management services – Goal: 50% by July 2023; <br />Number of unduplicated unsheltered homeless households who receive <br />case management services – Goal: 50% by July 2023; <br />3 Denominator for percentage to be based on census of unsheltered homeless residents in Redwood City to be <br />conducted by the RWC Coordinated Homeless Outreach Team in January 2023 <br />ATTY/AGR.2022.394/LifeMoves (Redwood City Coordinated Homeless Outreach) (Page 15 of 17)
