REV: 11-10-22 RL <br />Number and percentage of unsheltered individuals in Redwood City4 who <br />complete CES Assessment. Goal: 50% by March 2023; 75% by July 2023; <br />Number and percentage of unsheltered individuals participating in any kind of <br />health services (medical, mental health, substance disorder, or recovery <br />services. Goal: Goal: 25% by March 2023; 50% by July 2023; <br />Number and percentage of unsheltered individuals participating in mental health <br />and/or substance disorder and/or recovery services. Goal: 25% by July 2023; <br />50% by January 2024; and <br />Number and percentage5 of unsheltered individuals and households moving <br />into emergency or permanent housing (with breakdown of # to emergency <br />shelter and # to permanent housing) : <br />o First six months of 2023 – Goal: 30 individuals/households <br />o Second six months of 2023 – Goal: 20 individuals/households <br />o First six months of 2024 – Goal: 20 individuals/households <br />Reduction in the number of encampments in Redwood City or in the number of <br />individuals living in encampments in Redwood City (baseline to be established <br />by September 2022): <br />o by June 30, 2023 – 20% reduction <br />o by June 30, 2024 – 40% reduction <br />Consultant must submit annual reports, in addition to the monthly reports consistent <br />with the requirements of Section 6, Compensation, which summarize the achievement <br />of stated objectives at the end of each fiscal year covered by the term of this agreement. <br />2. Payment Terms <br />Consultant shall be paid as described in Section 6. <br />Prior to Consultant’s first advance payment request, Consultant shall provide City an <br />updated project budget. No advance payments will be made until the Homeless Services <br />Manager or their designee approves the updated project budget. Any subsequent <br />request for a modification to the updated budget must be submitted in writing and <br />approved by the Homeless Services Manager or their designee. <br />Upon approval of the updated project budget, Consultant shall begin providing City with <br />advance requests for payment as described in Section 6. City shall make payment within <br />thirty (30) days of Consultant’s payment request submission. After the first advance <br />payment, payment of advance monthly requests for payment shall be contingent upon <br />the Homeless Services Manager or their designee’s approval of the prior month’s invoice, <br />as described in Section 6.2. <br />Advance funds provided to Consultant that are not expended by the expiration of this <br />Agreement, including any amendments thereto, must be returned to the City within thirty <br />(30) days of City invoicing Consultant for the remaining funds. <br />4 Denominator for percentage to be based on census of unsheltered homeless residents in Redwood City to be <br />ATTY/AGR.2022.394/LifeMoves (Redwood City Coordinated Homeless Outreach) (Page 16 of 17)
