SCOPE OF SERVICES <br />Consultant shall conduct all remediation activities at 1071 El Camino Real, Redwood City, CA, <br />also known as Sequoia Station Shopping Center (“Property,” “Site”). <br />1. Tasks <br />Consultant shall complete the following tasks: <br />1.1. Pre Injection Activities <br />1.1.1. Health & Safety Plan (HASP) and Safety Meetings <br />Prior to any field activities, Consultant shall prepare a site-specific project HASP for <br />Public Works Director or designee review and approval. As part of the HASP, <br />Consultant shall also provide separate plans that discuss site security, traffic control <br />measures, equipment and materials laydown and staging areas, water hose route to <br />connect to fire hydrants for each area, Spill Response measures, storm water controls, <br />COVID-19 precautions, and a plan for sequencing the work activities in a manner that <br />reduces the footprint of the active work area, maintains safety, and minimizes business <br />disruptions as much as possible, including temporary no parking zones. Consultant may <br />not mobilize for site work without Public Works Director or designee review and <br />acceptance of the site-specific HASP and associated documentation described in this <br />agreement. <br />Consultant shall prepare a site-specific HASP and job safety analyses (JSAs) in <br />accordance with specified requirements. Consultant must provide a copy of the HASP <br />and JSAs to the Public Works Director or designee for review prior to mobilization. <br />Consultant must conduct one Health & Safety Loss Prevention Observation (LPO) (a.k.a <br />near miss reporting) per every five days of site activities during the project. Copies of <br />the completed LPOs are to be provided to the Public Works Director or designee within <br />one week of the observation. <br />Public Works Director or designee shall also prepare a site-specific HASP for work to <br />be completed at the site. The HASP will be reviewed and signed by all Consultant <br />personnel and others performing work at the site. A sign in sheet will be available each <br />day for each Consultant worker entering into the site. By signing the HASP, each <br />Consultant worker agrees that they understand and will follow the procedures as <br />outlined in the HASP. A copy of both HASPs must be present during all on-site activities. <br />All Consultant personnel shall have successfully completed the 40-hour OSHA and <br />annual 8-hour refresher trainings in accordance with 29 CFR 1910.120. All on-site <br />Consultant personnel shall maintain valid OSHA training certificates on-site at all times. <br />Consultant personnel tasked to work on this site shall be fully trained and qualified to <br />perform the work tasks that are assigned to them. All workers shall have at a minimum <br />evidence of undergoing a criminal background check, access to a substance abuse <br />program through their employer, current 40-hour OSHA HAZWOPER training along with <br />annual 8-hour Refresher training, current medical monitoring, and drug testing. <br />ATTY/AGR.2022.348/Regenesis Bioremediation Products (Page 11 of 34)
