REV: 10-05-2022 VR <br />Work activities may only be conducted by Consultant under the supervision of the Public <br />Works Director or designee. Consultant work activities may not be initiated until the <br />Public Works Director or designee is on-site. The Public Works Director or designee <br />reserves the right to stop work at any time for failure to comply with these or other <br />policies set forth in the site-specific HASP. <br />Consultant shall adhere to the following health and safety practices during all site <br />activities: <br />Level D personal protective equipment (PPE) is required for all site activities <br />including unloading of supplies and site cleanup; <br />Hearing protection is required during the use of any power equipment <br />producing noise levels above 85 dBA, including jack hammers, air knife, air <br />compressors or asphalt saws; <br />Construction vehicles must be equipped with functional back-up alarms; <br />Required work hazard safety zone set-back distances must be specified in <br />advance of work and adhered to at all times during the work; <br />Machinery should be de-energized when unattended and not in use; and <br />Ground-fault circuit interrupters (GFCI) are required for use with power tools. <br />Eyewash stations, first aid kits, and fire extinguishers shall be available on <br />site. <br />Consultant shall assume that Level D PPE shall be utilized during the project. Consultant <br />shall also assure that Level C PPE is available on-site at all times should an upgrade be <br />required. <br />Consultant shall be responsible for managing the inventory of injection chemicals <br />throughout the project and shall review the injection plan and work schedule prior to <br />beginning work to plan an up-front order and then cross-reference with the chemical <br />inventory for any subsequent orders. <br />Daily safety meetings shall be conducted and led by the Consultant at least twice per <br />day. A safety meeting shall be conducted by the Consultant prior to initiating any site <br />activities and during the work day (i.e. immediately after a meal or lunch break). The <br />Public Works Director or designee may require additional safety meetings during the <br />workday. <br />1.1.2. Preconstruction Kickoff Meeting <br />Consultant shall coordinate a pre-construction kickoff meeting with the Public Works <br />Director or designee, which shall occur at minimum 1 week prior to mobilization. This <br />meeting shall facilitate follow-up on any action items and confirm the project schedule <br />and site access. The meeting shall identify and specify project workflow and logistics. <br />Consultant shall coordinate two separate work crews working simultaneously at the Site, <br />with a total of two drilling rigs and two injection trailers. <br />1.1.3. Site Security, Traffic Control Measures, and Temporary No Parking Zones <br />The parking lot shall remain open to the public and operational during working hours. <br />Consultant shall provide all required traffic control measures and temporary no parking <br />zones for the injection areas and shall provide a plan in addition to the HASP for <br />ATTY/AGR.2022.348/Regenesis Bioremediation Products (Page 12 of 34)
