REV: 11-18-2022 MI <br />EXHIBIT “A” <br />SCOPE OF SERVICES AND FEE <br />Consultant shall provide the following services: <br />1) Professional short-term confidential counseling for approximately 125 personnel <br />(Sworn and Non- Sworn) and their eligible dependents living within the home who are <br />experiencing problems, which affect job performance and overall well-being. These <br />problems include, but are not limited to: mental, family and relationship conflicts, alcohol <br />and/or substance abuse, co-dependency, stress, behavioral, or emotional problems, <br />addictive behaviors, anger issues, anxiety / panic attacks, bereavement, critical incidents, <br />and depression. Counseling shall be provided by the Consultant’s network of mental <br />health professionals who have been vetted and evaluated for their experience and <br />expertise dealing with first responders and their families. Each employee and eligible <br />dependent will have access to ten (10) counseling sessions each annual year to address <br />problems that effect job performance and overall well-being. <br />2) Supervisor Referral: Provide consultations with supervisors regarding employees who <br />are unwilling to seek assistance on their own and/or any aspect of the employee support <br />services offered by the Consultant. Supervisors can make referrals for employees who <br />appear to need assistance although participation is still voluntary. <br />3) Follow-up counseling sessions for employees involved in Critical Incidents. ”Critical <br />incidents” can include, but are not limited to; line of duty deaths, officer-involved shootings, <br />in-custody deaths, response to child deaths, multiple casualty incidents (mass shootings, <br />multiple casualty auto accidents), witness to suicide, and any other incident where <br />personnel need assistance to process incidents in a healthy and productive manner. <br />4) Conduct an initial orientation for all levels, including management concerning <br />Consultant’s employee support services for no charge. A video link of the recorded <br />training will be provided for viewing after the initial orientation. Consultant will provide <br />electronic copies of brochures, flyers, support materials, etc. Consultant will attend one <br />health fair a year. <br />Fee Schedule <br />Consultant shall provide the services outlined above for a flat annual rate per the fee <br />schedule below. This flat rate will be paid in equal monthly installments.
