Provide services in the homeless encampments and in other locations where <br />individuals/households from the homeless encampments may be engaging in <br />services. <br />vii. Develop and maintain close working relationships with County partners (including <br />BHRS, Healthcare for the Homeless, Aging and Adult Services and other San <br />Mateo County Health programs), law enforcement, and other partners in order <br />to provide appropriate referrals. <br />viii. Make referrals to available short-term housing interventions, such as access to <br />an emergency shelter or other rapid homeless services, residential treatment <br />programs, and medical facilities as a step toward permanent housing. <br />ix. Conduct additional outreach to notify unsheltered individuals/households <br />if/when inclement weather is activated or that may be approaching to assist them <br />with accessing shelter services. <br />x. Provide family reunification resources and support services when appropriate <br />and if applicable. <br />b. Provide 1.0 FTE Housing Specialist, who will <br />i. Provide housing search and location support, including connection to property <br />managers and housing opportunities; <br />ii. Provide assistance with housing applications and securing documentation as <br />needed in coordination with outreach case managers; <br />iii. Provide support for securing stable income to maintain housing; and <br />iv. Provide housing move-in support. <br />5. Hotel respite program and housing subsidy services, to include: <br />a. Create a hotel respite pilot program offering short term access to hotel rooms as short- <br />term shelter (Hotel Respite Pilot Program) for clients identified by Grantee or their <br />subcontractors to provide stability and motivation to clients living on the street, including <br />people living in vehicles/RVs, or in encampments when 1) client is waiting for shelter; 2) <br />client is waiting for their housing unit to be ready; 3) client has a medical condition that <br />will get worse if they remain on the street or in their encampment; or 4) client has a <br />behavioral health condition that could benefit from respite to stabilize their condition and <br />allow them to be more willing to accept services. <br />b. Provide housing subsidies in the form of shallow rent assistance, Rapid Re-Housing <br />Funding, to be used in conjunction with existing housing programs that clients may be <br />eligible for (i.e. client was matched to a housing program but has an additional housing <br />subsidy need) and/or towards short term subsidies to bridge individuals to other long <br />term subsidy options. Housing subsidies may also include utility assistance, delinquent <br />rent assistance, or other assistance that will help maintain housing and housing stability <br />for formerly unsheltered residents – especially for long-time and chronically homeless <br />residents. <br />c. Continued development of guidelines and provide the draft guidelines to County for <br />review, feedback and approval. <br />6.
