In the event that specific agencies <br />included in the outreach team are changed, Contractor will submit details of the proposed <br />changes to the County for review and feedback <br />b. Implement a Collective Impact (CI) model that brings people together in a structured way <br />to achieve social change. This model will use a network of community members, <br />organizations, and institutions who advance equity by learning together, aligning, and <br />integrating their actions to achieve population and systems-level change. <br />c. Create a “by-name list” of unsheltered homeless residents in homeless encampments or <br />on the street, including people living in vehicles/RVs. <br />d. Cross-reference “by-name list” with HOT clients, clients pre-matched for Coordinated <br />Entry Services (CES) housing subsidies in the last six months, and encampment waste <br />services participants to determine homeless outreach partner existing <br />relationships/engagement. <br />e. Coordinate contacts with unsheltered individuals, focused on supporting individual case <br />plans <br />f. Assign a “lead” outreach worker/case manager for each individual on the “by-name list” <br />4. Encampment resolution outreach and engagement, to include: <br />a. Provide 1.0 FTE Program Director, who will: <br />i. Provide Project oversight; <br />ii. Monitor overall Project operations, including quality assurance and <br />improvement, consistency of services, ongoing training and supervision of staff, <br />partnerships with stakeholders, data tracking, and performance outcomes; <br />iii. Work closely with County on Project implementation, Project structure as it <br />relates to other outreach services in the community, and how this Project <br />interacts with other homeless system programs, such as Diversion and CES; and <br />iv. Recruit, retain, train, and supervise Project staff. <br />b. Provide 7.0 FTE Homeless Outreach Staff, who will: <br />i. Develop a homeless encampment map of encampments in Redwood City and <br />work with County to develop additional mapping strategies in Clarity. <br />ii. Provide outreach services and rapid response coordination on a set schedule, <br />including afterhours and weekend coverage to address immediate services or <br />interventions for unsheltered homeless households that may not be able to <br />access mainstream services during business hours. Project schedule may be <br />adjusted for other reasons, such as unanticipated needs or challenges arising, <br />subject to advance approval by County. <br />iii. Conduct standardized assessment to identify individual’s/household’s immediate <br />need(s) and short-term interventions, such as direct assistance for basic needs <br />(food, clothing, hygiene, etc.) or emergency calls for urgent medical needs. <br />iv. Build relationships with unsheltered individuals/households and service <br />providers based on trust and respect to establish a pathway to permanent <br />housing or connections to other homeless systems/services. <br />v.
