<br />e. Ensuring Project staff are aware of, or are connected to, and maintain knowledge of other
<br />supportive services available within or around San Mateo County, such as
<br />referral/application processes for shelters, housing resources, voucher and subsidy
<br />programs, senior housing, affordable housing, shared housing, substance use treatment
<br />programs, long‐term care/medical facilities, mental health services, Aging and Adult
<br />Services, public benefits (Social Security, CalFresh, General Assistance, health coverage
<br />programs, etc.) and/or other applicable resources.
<br />2. Adherence to the following outreach policies and procedures, to include:
<br />a. Services are provided consistently and in alignment with Housing First principles (i.e., not
<br />requiring people experiencing homelessness to address all of their problems including
<br />behavioral health problems, or to graduate through a series of services programs, before
<br />they can access housing).
<br />b. Consistent process for receiving, documenting, and responding to referrals and requests
<br />for service.
<br />c. Thorough, accurate, and timely documentation of all services in the San Mateo County
<br />Clarity HMIS system.
<br />d. Services are targeted to hardest-to-serve homeless individuals and families.
<br />e. Actively moving the greatest number of unsheltered individuals towards permanent
<br />housing.
<br />f. Implementation of a person-centered, strengths-based approach tailoring case
<br />management to each client.
<br />g. Collaboration between the RWC Homeless Outreach Team and safety net providers,
<br />homeless services, medical and behavioral health providers, substance use treatment
<br />programs, law enforcement, and other applicable partners and stakeholders.
<br />h. Client grievance procedures.
<br />i. Confidentiality policies and procedures.
<br />j. Safety and emergency protocol, and incident reporting.
<br />k. Close collaboration with County and the Homeless and Safety Net system, including non-
<br />profit and County and City programs that serve people experiencing homelessness and
<br />people at risk of homelessness.
<br />l. The Project will not screen out individuals/households based on criteria such as a
<br />minimum income threshold, employment, criminal history, disability, evidence of
<br />“motivation”, etc.
<br />m. Project is centered on a Housing First approach that values the needs of each individual.
<br />3. Establish and lead the Redwood City Coordinated Inter-Agency Homeless Outreach Team and
<br />associated strategy, to include:
<br />a. Establish the RWC Homeless Outreach Team led by lead agency Program Director
<br />overseeing coordination and deployment of outreach staff from multiple agencies
<br />focused on homeless outreach case management for unsheltered clients living in
<br />encampments or in vehicles/RVs. The RWC Homeless Outreach Team will include the
<br />following agencies: LifeMoves, Project WeHOPE, Downtown Streets Team, Street Life
<br />Ministries, and Nation’s Finest, in coordination with County-funded HOT staff, Behavioral
<br />Health and Recovery Services (BHRS) Crisis and Outreach (including full-time BHRS HEAL
<br />Clinician assigned to Redwood City), Redwood City Community Wellness and Crisis
<br />Response Clinician, Redwood City Police Department, Fair Oaks Community Center,
<br />Catholic Worker House, Street Medicine, Healthcare for the Homeless, Center on
<br />ATTY/AGR.2023.031/County of San Mateo (Agreement for Homeless Outreach) (Page 10 of 27)
