The Project funded by the Grant is to provide outreach and engagement services to <br />unsheltered people living in encampments and vehicles and/or other places not meant for human <br />habitation in Redwood City; provide funding for the establishment of Redwood City’s Coordinated <br />Inter-Agency Homeless Outreach Strategy Team (RWC Outreach Team) and strategy implementation <br />to coordinate outreach services and case management across multiple entities and organizations; <br />services will include intensive homeless outreach at encampments using a by-name list approach, <br />deployment of weekly waste services, hotel respite services, and provision of housing subsidies. <br />Grantee shall be responsible for ensuring the Project, through Grantee and/or its service providers, <br />provides services consistent with the requirements and parameters specified herein. <br />a. Population and Area to be Served <br />The Project will provide services to individuals who are, or appear to be, unsheltered or experiencing <br />homelessness (living outdoors, in vehicles, or in other places not meant for human habitation) in the <br />City of Redwood City, with a specific focus on individuals living in encampments and vehicles/RVs. <br />b. Project Services to be Provided <br />1. Program Oversight, to include: <br />a. Daily Project oversight by RWC Homeless Outreach Program Director to ensure that <br />coordination of inter-agency homeless outreach teams, outreach services, coordination <br />of outreach to persons in encampments, hotel respite services, and housing subsidy <br />services provided under this Agreement are in accordance with, and adhere to, the <br />Homeless Outreach Team (HOT) model, which includes HOT processes, guidelines, and <br />criteria for all program components of homeless outreach. <br />b. Working with County to strategize on connecting unsheltered individuals/households <br />with/to shelter and/or permanent housing; tracking the overall Project impact; <br />implementing changes based on continuous learning; and strategizing how to best deploy <br />program/services to meet Project goals. <br />c. Training, supervising, and supporting RWC Outreach Team to ensure they are (i) trained <br />to work with hard‐to‐reach and hard‐to‐serve clients who may otherwise be <br />disconnected from mainstream and homeless system services and supports; (ii) trained <br />on who to implement best practices of engagement in a trauma-informed and culturally <br />competent manner that respects the individual’s right to accept or deny services; and (iii) <br />able to consult, as needed, with experienced staff who have clinical expertise to <br />brainstorm additional methods for engaging and serving clients. <br />d. Providing on an ongoing basis comprehensive staff trainings with training topics to <br />include: best practices to working with homeless individuals who have complex needs; <br />safety protocol and procedures; Housing First model/approach (defined below); <br />motivational interviewing; engaging hard-to-serve clients; job functions and <br />responsibilities; emergency response protocol; reporting of suspected child <br />abuse/neglect and elder abuse; strategies on assisting clients with exploring housing <br />plans (including housing alternatives other than formal homeless programs, such as <br />family reunification, shared housing, etc.); and Homeless Management Information <br />System (“HMIS”) data entry and service documentation. <br />ATTY/AGR.2023.031/County of San Mateo (Agreement for Homeless Outreach) (Page 9 of 27) <br />DocuSign Envelope ID: 6F079F33-00E9-42D4-A508-A3B82FC663F4
