Assist individuals/households with becoming “document ready” and with processes for <br />accessing various housing programs listed above in section 6(i), including direct support <br />with obtaining identification and verification documents from agencies such as the Social <br />Security Administration and the California Department of Motor Vehicles. <br />l. Encourage use of shelters as a temporary place to stay while individual/household <br />pursues permanent housing. This includes the Hotel Respite Program for those who are <br />eligible and selected for that program, and also includes homeless shelter programs <br />located throughout the County. <br />m. Plan for and implement the transition of care and services to other support services or <br />case managers when individuals/households successfully transition into shelter or other <br />housing. <br />n. Identify non-housing related services that individuals/households are already connected <br />to, or enrolled with, and coordinate communication and co-case management to facilitate <br />access, care, and transparency on roles and responsibilities in case planning toward <br />permanent housing. <br />o. Monitor case progress, reassessment and revision of case plans and strategies on an as <br />needed basis. <br />p. Maintain timely and accurate case file management and maintenance in HMIS Clarity to <br />include pertinent individual/household information, case management activity <br />documentation, and progress notes. <br />q. Follow safety and emergency response protocol, including contacting emergency <br />responders when necessary and reporting critical incidents. <br />7. Ongoing learning and evaluation activities, to include: <br />a. Partner with County to collect data to measure program impact and for use on Project <br />design and/or improvements. <br />b. Using information from outreach services and/or specialized count activities to maintain <br />current information on the number of people who appear to be experiencing <br />homelessness, with cadence and approach to conducting these counts to be developed <br />in coordination with County. <br />c. Conduct ongoing tracking of service delivery, with cadence, format, and scope of such <br />tracking to be developed in coordination with County. <br />d. Work closely with County to conduct other field surveys and/or other information- <br />gathering initiatives. <br />e. Participate in Working Together to End Homelessness innovation learning circles, <br />evaluation activities, and meetings with the County, County’s evaluation partner, and <br />other stakeholders. <br />8. Quality assurance control and continuous quality improvements, to include: <br />a. Provide ongoing training, coaching, and review of services and data to ensure quality of <br />services, consistency of services, and adherence to policies and procedures. <br />b. Provide ongoing review and documentation of services, in a form and manner to be <br />developed in coordination with County. <br />c.
