Partner with County BHRS to identify ways to refine structures and systems for <br />coordination in the field to provide for training, consultation, escalation of cases and <br />coordinated planning including, when appropriate, to the involuntary treatment system. <br />e. Develop and implement strategies to enlist and support volunteers to support positive <br />connections with unsheltered individuals/households. <br />9. Additional Responsibilities: <br />a. Provide services that are culturally appropriate to the populations served. <br />b. Respond to rapid response requests from County and other partners identified by the <br />County, including participating in the Outreach Request Application and any other County <br />rapid response procedures. <br />c. Maintain and, as needed, update policies, procedures, and tools for staff to align Housing <br />First principles and enable outreach staff to identify and serve the hardest-to-serve. <br />d. Maintain timely, accurate client records of all clients served in the San Mateo County <br />Clarity/HMIS database (see Exhibit D). All client records will be entered into Clarity in a <br />timely manner (i.e., during or as soon as possible after the services). <br />e. If County requests, participate in expanded or new Clarity functionality that includes <br />additional data on people served. <br />f. Report to County within 24 hours all critical incidents, including death, homicide, suicide <br />or suicide attempt, assaults (to another client or staff) and/or any other significant <br />incident involving any HOT client or staff. <br />g. Participate in County homeless system meetings and the CES. <br />h. Participate in point-in-time homelessness counts and surveys. <br />10. Grantee matching contributions to the Project <br />a. As a condition of receiving the Grant, Grantee shall contribute the following amounts to <br />the Project from non-Grant funds (Matching Funds): <br />i. $790,856 <br />ii. Grantee shall provide documentation to County sufficient to substantiate its <br />Matching Funds. <br />11. County will: <br />a. Have the option to add or modify related services, and adjust costs, as agreed upon in <br />writing by County and Grantee, provided that, under no circumstances shall County’s total <br />fiscal obligation exceed the Grant amount. <br />b. Have the option, in consultation with Grantee and upon advance written notice, to <br />schedule, modify the number, frequency, or structure of collaborative meetings based on <br />shifting community needs. Collaborative meetings are meetings with County programs, <br />homeless service providers and other stakeholders. <br />c.
