Page 1 Grant Agreement Sand Hill Foundation is pleased to award a 2-year grant to the City of Redwood City acting as fiscal agent for Redwood City Together (hereafter referred to as "grantee") in the amount of $100,000 to be used for support of Redwood City Together’s ACT CREATE ENGAGE (ACE) Transition Age Youth Program. Payments of $50,000 will be made in November 2022 and November 2023,. The grant period will be December 1, 2022, to April 30, 2025. While we hope this grant helps Redwood City Together meet the outcomes provided below, we recognize that change is sometimes necessary and this grant is not conditional on these exact outcomes being achieved: •Ensure access to youth development programs in the City of Redwood City and surrounding communities to support mentorship pathways to workforce development and self-empowerment. •Organize and co-lead monthly ACE Plan Committee meetings with Redwood City Together for the purposes of status/reporting, describe measurable successes, share challenges, present opportunities, request any additional support needed, and update next steps and timelines. •Identify and work with Community Based Organizations (CBO's) to help implement the ACE Plan and meet program goals. •Develop and provide weekly coaching and mentorship programs for youth in vulnerable communities. •Youth ages 12-24 and their families will have opportunities to serve on and co-lead Youth/CBO Advisory spaces to shape programming, services, and policies for TAY programs. •Increase Youth Serving Programs and Amenities in Redwood City •Support, plan, facilitate and partner with our local school districts, healthcare partners, business stakeholders and community-based organizations to host quarterly convening's to discuss ways to support and reduce barriers for youth to participate in local activities and programs.
