Page 1 <br />Grant Agreement <br />Sand Hill Foundation is pleased to award a 2-year grant to the City of Redwood City acting as <br />fiscal agent for Redwood City Together (hereafter referred to as "grantee") in the amount of <br />$100,000 to be used for support of Redwood City Together’s ACT CREATE ENGAGE (ACE) <br />Transition Age Youth Program. Payments of $50,000 will be made in November 2022 and <br />November 2023,. The grant period will be December 1, 2022, to April 30, 2025. <br />While we hope this grant helps Redwood City Together meet the outcomes provided below, we <br />recognize that change is sometimes necessary and this grant is not conditional on these exact <br />outcomes being achieved: <br />•Ensure access to youth development programs in the City of Redwood City and <br />surrounding communities to support mentorship pathways to workforce development <br />and self-empowerment. <br />•Organize and co-lead monthly ACE Plan Committee meetings with Redwood City <br />Together for the purposes of status/reporting, describe measurable successes, share <br />challenges, present opportunities, request any additional support needed, and update <br />next steps and timelines. <br />•Identify and work with Community Based Organizations (CBO's) to help implement the <br />ACE Plan and meet program goals. <br />•Develop and provide weekly coaching and mentorship programs for youth in <br />vulnerable communities. <br />•Youth ages 12-24 and their families will have opportunities to serve on and co-lead <br />Youth/CBO Advisory spaces to shape programming, services, and policies for TAY <br />programs. <br />•Increase Youth Serving Programs and Amenities in Redwood City <br />•Support, plan, facilitate and partner with our local school districts, healthcare partners, <br />business stakeholders and community-based organizations to host quarterly convening's <br />to discuss ways to support and reduce barriers for youth to participate in local activities <br />and programs. <br />The grantee agrees to the following: <br />•Grantee shall use the foundation’s online grants management portal to submit fiscal and <br />narrative interim and final reports on the use of these funds within the last month at the <br />REV: 08-01-2023 RL <br />ATTY/AGR.2023.194/Sand Hill Foundation (Act Create Engage Transition Age Youth Program) (Page 2 of 4)
