The collaborative formally initiates a <br />process to develop a shared vision and <br />goals for community health <br />improvement that prioritizes equity, <br />based on input and engagement from <br />multi-sector partners and the <br />community. <br /> <br />• ACH members will review <br />CCCS Neighborhood Action <br />Team work to date; identify <br />community needs and <br />assess any gaps or areas of <br />improvement in current <br />community-based and CCCS <br />services (through <br />community needs <br />assessment if needed); <br />develop a violence <br />prevention plan <br />incorporating youth and <br />community voice that <br />includes evidence-based and <br />community developed <br />• The ACH will review the <br />violence prevention plan to <br />ensure the collective has the <br />capacity, infrastructure, <br />revenue, and consensus in <br />place to succeed.
