REV: 12-08-23 SK EXHIBIT “A” SCOPE OF SERVICES AND FEE ON-CALL AS-NEEDED POTHOLING SERVICES Upon request by Task Order from the City, the Consultant shall perform for the City on- call, as-needed potholing services for the City’s various projects and other related construction activities that require potholing location services to ensure the safety and accuracy of the City’s utility infrastructure and urban facilities system. Specific locations shall be on an on-call, as-needed basis, and will be determined by the City. The potholing services work include, but not limited to, the following: 1.Implement traffic control such as standard lane closures. 2.Pothole by vacuum excavation at the top of the designated pipe locations. 3.Remove and properly dispose of debris. 4.Compact and backfill potholes per City/County Standards and requirements. 5.Restore pavement surfaces with 4” minimum asphalt concrete or match existing pavement thickness. Match pavement color. 6.Provide all required labor, materials, and equipment to conduct the potholing not specifically addressed above but necessary for project delivery such as labor, reporting, travel, traffic control, permitting, potholing, debris disposal, pavement restoration, technical supervision, equipment, materials, taxes, insurance and all other incidental charges and all other expenses. 7.Document test data and observations through daily and end-of-job summary reports. Daily reports shall be submitted to the City on a daily basis. Daily reports should describe the activities performed at the time of field services, and the locations and results of the potholing investigations performed. 8.Submit Field Investigation Report with pothole location maps. Indicate the following data for each pothole: a.Profile, material(s) and thickness(es) of existing pavement. b.Depth of all existing utilities from top of pavement to the top of pipe(s). c.Distance of potholed utilities from face of curb/edge of pavement and to the center of nearest manhole (if applicable). d.Type and size of existing utilities potholed. 9.Contact all the applicable agencies and comply with all the necessary potholing permitting requirements, including but not limited to: a.Mark out boundaries, contact Underground Service Alert (USA) to mark existing utilities prior to potholing activities. b.Comply with National Pollutant Discharge Elimination System (NPDES) requirements. c.Comply with the City of Redwood City backfilling requirements. d.Obtain and comply with the City of Redwood City construction permit requirements. The City’s fee shall be waived. The work shall be performed by qualified personnel with valid professional licenses and certifications.
