REV: 12-08-23 SK <br />EXHIBIT “A” <br />SCOPE OF SERVICES AND FEE <br />ON-CALL AS-NEEDED POTHOLING SERVICES <br />Upon request by Task Order from the City, the Consultant shall perform for the City on- <br />call, as-needed potholing services for the City’s various projects and other related <br />construction activities that require potholing location services to ensure the safety and <br />accuracy of the City’s utility infrastructure and urban facilities system. Specific locations <br />shall be on an on-call, as-needed basis, and will be determined by the City. The <br />potholing services work include, but not limited to, the following: <br />1.Implement traffic control such as standard lane closures. <br />2.Pothole by vacuum excavation at the top of the designated pipe locations. <br />3.Remove and properly dispose of debris. <br />4.Compact and backfill potholes per City/County Standards and requirements. <br />5.Restore pavement surfaces with 4” minimum asphalt concrete or match existing <br />pavement thickness. Match pavement color. <br />6.Provide all required labor, materials, and equipment to conduct the potholing not <br />specifically addressed above but necessary for project delivery such as labor, <br />reporting, travel, traffic control, permitting, potholing, debris disposal, pavement <br />restoration, technical supervision, equipment, materials, taxes, insurance and all <br />other incidental charges and all other expenses. <br />7.Document test data and observations through daily and end-of-job summary <br />reports. Daily reports shall be submitted to the City on a daily basis. Daily reports <br />should describe the activities performed at the time of field services, and the <br />locations and results of the potholing investigations performed. <br />8.Submit Field Investigation Report with pothole location maps. Indicate the following <br />data for each pothole: <br />a.Profile, material(s) and thickness(es) of existing pavement. <br />b.Depth of all existing utilities from top of pavement to the top of pipe(s). <br />c.Distance of potholed utilities from face of curb/edge of pavement and to <br />the center of nearest manhole (if applicable). <br />d.Type and size of existing utilities potholed. <br />9.Contact all the applicable agencies and comply with all the necessary potholing <br />permitting requirements, including but not limited to: <br />a.Mark out boundaries, contact Underground Service Alert (USA) to mark <br />existing utilities prior to potholing activities. <br />b.Comply with National Pollutant Discharge Elimination System (NPDES) <br />requirements. <br />c.Comply with the City of Redwood City backfilling requirements. <br />d.Obtain and comply with the City of Redwood City construction permit <br />requirements. The City’s fee shall be waived. <br />The work shall be performed by qualified personnel with valid professional licenses and <br />certifications.
