REV: 08-13-24 LF <br />EXHIBIT “A” <br />SCOPE OF SERVICES AND FEE <br />Scope of Services <br />The Contractor shall provide the following services: <br />Hazardous Waste Hauling: The Contractor must provide inventorying, packaging, <br />removal, labeling, manifesting, loading, transporting and disposal of hazardous <br />waste/materials on a requested basis from the Public Works Wastewater Superintendent <br />or their designee. The work to be done under this Agreement includes, but is not limited to, <br />providing all labor, materials, equipment, incidentals and related items necessary to <br />complete the work. Additionally, the Contractor shall: <br />Confirm all requested services to the Public Works Wastewater Superintendent or <br />their designee within twenty-four (24) hours of a request and provide a written <br />quote for services required to address the job based on pricing in the fee schedule <br />below within 48 hours unless otherwise approved by the Public Works Wastewater <br />Superintendent or their designee. The requested service must begin within five (5) <br />business days unless otherwise approved by the Public Works Wastewater <br />Superintendent or their designee. <br />Contractor is required to perform characterization of all waste streams using <br />professional knowledge to the waste and to the processes generating to the waste, <br />and/or other methods of chemical analysis including but not limited to Toxicity <br />Characteristic Leaching Procedure (TCLP). The Contractor must have access to <br />the services of a California State-Certified environmental laboratory equipped to <br />perform waste characterization analyses mandated by the Code of Federal <br />Regulations 40 and Code of California Regulations Title 22, and in accordance <br />with USEPA Solid Waste 846 test methods of evaluation of solid wastes, <br />physical/chemical methods. <br />Contractor shall be responsible for consolidating all wastes into the most cost- <br />effective sized container and consolidation most beneficial. The Contractor shall <br />have the capability to supply waste collection containers of various sizes and <br />labels. <br />Should hazardous materials be released during the performance of services under <br />this Agreement, through no fault of the City, the Contractor will be responsible for <br />all costs associated with the satisfactory remediation of the incident. This will <br />include the cost of all labor and materials as well as any actual damages incurred <br />to the City and harm caused to staff and visitors of the City site. The remediation <br />efforts shall be performed to the satisfaction of the Public Works Wastewater <br />Superintendent or their designee. The Contractor shall submit a written incident <br />report to the Public Works Wastewater Superintendent or their designee and any <br />designated entities within 48 hours of the occurrence of any spill of waste caused <br />ATTY/AGR.2024.129/Advanced Chemical Transport dba ACTenviro (Hazardous Waste Hauling) (Page 11 of 14)
