Notification shall include the reason and <br />the possible duration of the absence. <br /> <br />Employees are responsible for keeping the Department informed of <br />their continuing condition and probable date of return to work. <br /> <br />For absences in excess of three (3)five (5) or more consecutive work <br />days or duty shifts, the Fire Chief may require a physician's <br />statement from an employee who applies for additional sick leave, <br />or whatever investigation into the circumstances which appear <br />warranted, before taking action on the request. <br /> <br />18.2 Temporary Modified Duty Assignments –Industrial and Non-Industrial <br /> <br />18.2.1 Subject to operational considerations, the Department may identify <br />temporary modified duty assignments for employees who have an <br />injury or medical condition resulting in temporary work limitations <br />or restrictions. <br /> <br />18.2.2 Employees may request a temporary modified duty assignment for <br />short-term injuries or illnesses. The request should include a <br />certification from the treating physician containing the nature and <br />scope of limitations and/or work restrictions and a statement that <br />the employee can safely perform the duties of the temporary <br />modified duty assignment. The duty limitations specified by the <br />treating physician must be strictly adhered to when making work <br />assignments. The department routinely requires periodic updated <br />reports, which should also contain modified duty information. For <br />employees whose modified duty arrangement calls for less than the <br />regular number of hours worked per payroll period, the hours <br />actually worked will be shown as regular time on the time card, and <br />the remaining hours (the difference between hours actually worked <br />and hours normally scheduled) charged to the appropriate leave. <br /> <br />18.2.3 Reserved. <br /> <br />18.2.4 Modified duty assignments should be such that there is minimum <br />risk of further injury or aggravation. <br /> <br />18.2.5 Modified duty shall not continue indefinitely. No position in the <br />Department will be created or maintained as a temporary modified <br />duty assignment. Temporary modified duty assignments are a <br />management prerogative and not an employee right.
