<br />67 <br /> <br />If the benefits provided under the federal or state act exceed the benefits <br />provided hereunder in each category of coverage, the City shall be under no <br />further obligation to make any contributions. <br /> <br />19.5 Life Insurance <br /> <br />The City shall provide to eligible employees basic life insurance coverage of two <br />thousand dollars ($2,000), and Accidental Death and Dismemberment (ADD) <br />coverage of fifteen thousand dollars ($15,000), with the City paying one hundred <br />percent (100%) of the premium. <br /> <br />The City shall continue to offer to eligible employees additional life insurance <br />equal to one and one-half (1.5) times the employee’s annual salary, at a 60/40 <br />premium contribution split between the City and the employee respectively. <br /> <br />19.6 Vision Care <br /> <br />The City shall continue to contract with Vision Service Plan (VSP) or a comparable <br />vision care provider to provide Vision Care Plan B benefits for I.A.F.F. employees <br />and their dependents. The Vision Service Plan B provides for an exam every <br />twelve (12) months, lenses every twelve (12) months if needed, and frames <br />every two (2) years if needed. There is to be no deductible for employees, but a <br />twenty dollar ($20.00) per person deductible will apply to dependents each time <br />benefits are available and will be paid by the employee. The City will pay ninety <br />percent (90%) of the vision insurance premium for eligible employees and <br />dependents. <br /> <br />19.7 Long Term Disability Insurance <br /> <br />The City will reimburse twenty-nine dollars and fifty cents ($29.50) per month <br />per employee towards the cost of Long Term Disability insurance. <br /> <br />SECTION 20 – BENEFITS DURING STRIKE TEAM ASSIGNMENT <br /> <br />Any employee on State or Federal Assignment shall remain an e mployee of the City and <br />shall retain all rights and benefits as provided to City employees covered by this <br />agreement, notwithstanding State or Federal reimbursements to the City. <br /> <br />SECTION 21 - SAFETY <br /> <br />21.1 Both the City and the Association shall expend every effort to see to it that work <br />is performed with a maximum degree of safety, consistent with the requirement <br />to conduct efficient operations. <br /> <br />6.A. - Page 78 of 99 <br />85
