RE: 09-19-24 LF Fees: Task 1: Initial Preventative Maintenance Inspections City shall pay Contractor on a lump- sum basis to provide initial preventative maintenance inspections, per the fee schedule below. Facility Flat-Fee Fair Oaks Community Center $3,734 Red Morton Community Center $3,734 Total $7,468 Task 2: As-Needed Preventative Maintenance and Emergency Response Services City shall pay Contractor on an as-needed basis to provide preventative maintenance and emergency response services per the fee schedule and other charges described below, in an amount not to exceed Ninety-Two Thousand Five Hundred Thirty-Two Dollars ($92,532). On-Site Service Remote Online Service Monday – Friday 7:30 AM – 3:30 PM (Regular Time/RT) $224/hr $224/hr Monday – Friday 3:31 PM – 7:29 AM (Overtime/OT) $324/hr $324/hr Saturday 24 Hours per day (Overtime/OT) $324/hr $324/hr Sunday 24 hours per day (Double Time/ DT) $424/hr $424/hr Holidays 24 hours per day (Double Time/DT) $424/hr $424/hr •OT/DT travel time from place of residence to jobsite will be applied to on-site service. •A trip charge of $150 will be applied to every on-site service call requested. •A phone/equipment charge of $35.00 for the first hour and $25.00 for each additional hour shall be applied for each remote online service call. •Rates are based on a maximum of eight (8) hours per day. Rates will be billed at 1 ½ times per hour for hours worked over eight (8) hours per day. •There will be a two-hour minimum charge for each on-site service call and 15 minute minimum plus first hour phone equipment charges for each remote online service call. Holiday time shall only be billed for observed holidays as outlined in Exhibit “B”.
