<br />31 <br /> <br />health statements from their physician upon reasonable <br />request. <br /> <br />9.6.2.3 Prior to the establishment of a specific date for return to duty, <br />the employee shall submit to the Personnel Officer a notice of <br />intention to return to duty, accompanied by their physician's <br />statement certifying that the employee is medically qualified <br />to assume regular duties and responsibilities. <br /> <br />9.6.2.4 The Personnel Officer or their designee may designate the <br />specific beginning and ending dates within the pay period <br />requested by the employee to meet the needs of the employee <br />and the City. <br /> <br />9.6.3 The employee on leave shall be returned to an equivalent position within <br />their classification. <br /> <br />9.6.4 A pregnancy leave, absent physical disability, is granted without pay for <br />the duration of the leave. The employee may elect to continue medical <br />and dental insurance coverage for up to one (1) year during that leave at <br />their own expense. <br /> <br />The combination of pregnancy leave and parental leave shall not exceed <br />one (1) year. <br /> <br />9.7 Leave for Pregnancy Disability <br /> <br />9.7.1 Employees are entitled to use personal illness and injury leave for <br />disabilities caused or contributed to by pregnancy, miscarriage, childbirth, <br />and recovery therefrom on the same terms and conditions governing <br />leaves of absence for other illness or medical disability, and in accordance <br />with the City’s policy on Pregnancy Disability Leave, in effect as of the <br />date of this agreement, and as subsequently amended subject to notice to <br />the Association and the opportunity to meet and confer where required by <br />law. <br /> <br />Family Medical Leave and Pregnancy Disability Leave will run <br />concurrently with Parental leave as permitted by law. <br /> <br />9.8 Parental Leave of Absence <br /> <br />An employee may be granted, at the Police Chief’s discretion, a leave of absence <br />without pay to fulfill parenting responsibilities for up to one (1) year following the <br />child’s birth or one (1) year following the filing of application for adoption and <br />actual arrival of a child in the home. <br /> <br />The combination of pregnancy leave and parental leave shall not exceed one (1) <br />year. <br />6.I. - Page 43 of 84 <br />273
