REV: 02-04-25 LF <br />Exhibit “A” <br />SCOPE OF SERVICES AND FEE <br />The City owns eleven (11) hydraulic elevators, three (3) wheelchair lifts, and one (1) <br />freight lift. <br />1) Contractor Requirements <br />Contractor shall satisfy and maintain the following criteria: <br />•Maintain skilled and experienced mechanics and adjustors under its direct <br />employment and supervision. <br />•Certify that a member of its supervisory personnel regularly engaged in <br />inspection and supervision will visit each listed elevator at least semi-annually to <br />observe the quality of maintenance and to make certain that the quality of <br />maintenance meets the specified standards. The supervisor shall report his/her <br />findings in writing to the Public Works Superintendent- Facilities. <br />•Obtain repair parts within a maximum of two (2) days, to minimize elevator <br />downtime. <br />•Have a Call Center that is available 24 hours per day, 365 days per year, which <br />can dispatch technicians to the site within the specified time allowed. <br />2) Preventative Maintenance Requirements <br />Any work not specifically listed below, but which is needed to make the maintenance <br />complete within the intent of this specification, shall be performed without additional <br />cost. <br />Contractor shall: <br />•Systematically examine, adjust, clean, and lubricate the elevators in accordance <br />with the manufacturer's specified preventative maintenance schedule. <br />•Assign primary and backup technician(s) to the account. An account <br />representative shall also be assigned to be the contact for communications <br />regarding the Agreement. <br />•Set up specific days that preventative maintenance shall occur each month and <br />notify City of that schedule. If the service person is called away during the <br />scheduled maintenance and cannot complete the work, they will return the next <br />working day. <br />•Repair or replace: <br />i) Machine drive sheave or drum, drive sheave or drum shaft bearing, brake <br />pulley, brake coil, brake contact, lungs and component parts, drum buffers, <br />ATTY/AGR.2025/Amend. no.1/Star Elevator, Inc.
