REV: 02-04-25 LF Exhibit “A” SCOPE OF SERVICES AND FEE The City owns eleven (11) hydraulic elevators, three (3) wheelchair lifts, and one (1) freight lift. 1) Contractor Requirements Contractor shall satisfy and maintain the following criteria: •Maintain skilled and experienced mechanics and adjustors under its direct employment and supervision. •Certify that a member of its supervisory personnel regularly engaged in inspection and supervision will visit each listed elevator at least semi-annually to observe the quality of maintenance and to make certain that the quality of maintenance meets the specified standards. The supervisor shall report his/her findings in writing to the Public Works Superintendent- Facilities. •Obtain repair parts within a maximum of two (2) days, to minimize elevator downtime. •Have a Call Center that is available 24 hours per day, 365 days per year, which can dispatch technicians to the site within the specified time allowed. 2) Preventative Maintenance Requirements Any work not specifically listed below, but which is needed to make the maintenance complete within the intent of this specification, shall be performed without additional cost. Contractor shall: •Systematically examine, adjust, clean, and lubricate the elevators in accordance with the manufacturer's specified preventative maintenance schedule. •Assign primary and backup technician(s) to the account. An account representative shall also be assigned to be the contact for communications regarding the Agreement. •Set up specific days that preventative maintenance shall occur each month and notify City of that schedule. If the service person is called away during the scheduled maintenance and cannot complete the work, they will return the next working day. •Repair or replace: i) Machine drive sheave or drum, drive sheave or drum shaft bearing, brake pulley, brake coil, brake contact, lungs and component parts, drum buffers, ATTY/AGR.2025/Amend. no.1/Star Elevator, Inc.
