REV: 01-16-24 MI <br />EXHIBIT “A” <br />SCOPE OF SERVICES AND FEE <br />The City owns eleven (11) hydraulic elevators, three (3) wheelchair lifts, and one (1) <br />material lift. <br />1) Contractor Requirements <br />Contractor shall satisfy and maintain the following criteria: <br />Maintain skilled and experienced mechanics and adjustors under its direct <br />employment and supervision. <br />Certify that a member of its supervisory personnel regularly engaged in inspection <br />and supervision will visit each listed elevator at least semi-annually to observe the <br />quality of maintenance and to make certain that the quality of maintenance meets <br />the specified standards. The supervisor shall report his/her findings in writing to <br />the Public Works Superintendent- Facilities. <br />Obtain repair parts within a maximum of two (2) days, to minimize elevator <br />downtime. <br />Have a Call Center that is available 24 hours per day, 365 days per year, which <br />can dispatch technicians to the site within the specified time allowed. <br />2) Preliminary Inspections <br />Contractor has conducted an industry-standard visual examination of the listed <br />elevators and lifts. An examination of this nature is insufficient to determine the <br />condition of the equipment or to what extent contractual performance standards are <br />being met. As such, Contractor shall have 45 days from the Effective Date of the <br />Agreement to conduct an in-depth examination of the equipment and to submit a <br />proposal for the recommended repairs that will be reasonably necessary in order for <br />Contractor to provide full coverage of the equipment under the Agreement. Such <br />proposal shall include a recommended scope of work and price for each elevator or <br />lift. <br />3) Preventative Maintenance Requirements <br />Any work not specifically listed below, but which is needed to make the maintenance <br />complete within the intent of this specification, shall be performed without additional <br />cost. <br />Contractor shall: <br />Systematically examine, adjust, clean, and lubricate the elevators in accordance <br />with the manufacturer's specified preventative maintenance schedule. <br />ATTY/AGR.2024.006/Star Elevator, Inc.
