REV: 02-12-25 LF <br />F. Chain of Custody Procedures <br />All samples to be analyzed by the Consultant will be accompanied by a chain of custody <br />(COC) form. In the event COC information is in error, or there is a question or clarification <br />regarding information on the COC, Consultant will contact the Public Works Director, or <br />designee, (“City Staff”) immediately and prior to proceeding with the analysis. <br />A completed copy of the COC, signed by Consultant, will accompany all sample reports. <br />G. UCMR Analyses <br />The City is required to collect samples under the UCMR5. Samples collected for UCMR5 <br />must be analyzed according to UCMR guidelines by a USEPA approved laboratory. <br />H. Analytical Report for Samples <br />In addition to any electronic data transfer requirements, Consultant will email an electronic <br />PDF copy of the sample analytical results to the project contact(s) associated with the <br />account or sub-account. Separate accounts or sub-accounts will be based on the <br />following PWS divisions: <br />Drinking Water <br />Recycled Water <br />Wastewater (includes: lagoons, creeks, sewer, soils) <br />A copy of the associated COC form shall be included with each report. The cost of these <br />PDF reports must be included in the unit cost for each analysis. <br />Timely electronic reports of sample results must be provided to City Staff. All analytical <br />reports shall be provided by the Consultant within seven calendar days of completing the <br />analysis. For drinking water samples that are collected weekly, Consultant shall deliver <br />completed reports by the 5th of the following month in which they were collected in order <br />for City Staff to submit monthly reports by the 10th to the SWRCB DDW. <br />City Staff may request expedited analyses and reporting as needed to meet regulatory <br />reporting due dates. <br />Consultant will be required to upload drinking water and recycled water sample results <br />online to the City’s WaterTrax account. <br />The PDF reports are separate and in addition to the WaterTrax and EDT reporting. <br />II. FEE <br />Payment will be made within thirty (30) days of receipt of an accurate invoice. Payment <br />will be made no more frequently than monthly. Invoices will also include the appropriate <br />project number and description. Invoices will either be emailed to pws- <br />adminsupport@redwoodcity.org or mailed to: Public Works Services Department, Attn: <br />(Division Name), 1400 Broadway, Redwood City, CA 94063. <br />ATTY/AGR.2025.023/Alpha Analytical Laboratories, Inc.
