Complete budget and activity reports as demonstrated in Exhibit A and Exhibit B and <br />submit them to the County Superintendent on or before the dates listed below: <br /> <br />TRIMESTER BUDGET & ACTIVITY <br />REPORTS DUE DATE <br />1st - 9/1/24 – 12/31/24 1/17/25 <br />2nd - 1/1/25 – 3/31/25 4/18/25 <br />3rd - 4/1/25 – 6/18/25 6/30/25 <br /> <br />Activity Report template and instructions will be reviewed with Agency representatives on an <br />as-needed basis. <br />D. Complete end of the year budget and activity reports as demonstrated in Exhibit A and <br />Exhibit B and submit them to the County Superintendent on or before June 30, 2025 for <br />the 2024-2025 school year. <br />E. Gather two consecutive days of student travel data in spring 2025. Submit during the <br />February through May evaluation period in spring 2025. <br />F. Promote parent/caregiver survey during the spring 2025 evaluation period. <br />G. Participate in two of the following Countywide events: International Walk to School Day <br />(October 9, 2024); Ruby Bridges Walk to School Day (November 14, 2024); Earth Week <br />(April 21-25, 2025); National Bike to School Day (May 7, 2025)l and/or Bike Month (May <br />2025). <br />H. Check-in regularly with SMCOE SRTS Coordinator. <br /> <br />I. When sending out information about funded projects and promoting County-wide <br />events, include the following: <br />1) Event dates and a description of event activities <br />2) Contact information for Agency’s local SRTS coordinator <br />3) Suggested walk and bike route information to the extent practical <br />4) Source of funding is the San Mateo County Transportation Authority (TA). <br />5) Include the TA logo and the SMCOE SRTS logo, which will be provided <br /> <br />J. Maintain records for program review, evaluation, audit and/or other purposes and <br />make them available to persons authorized by the State, including the County <br />Superintendent, and/or grantor agencies. Should the contractual relationship terminate, <br />all original records will be provided to the County Superintendent within ten (10) <br />calendar days of the termination. <br /> <br />K. Maintain all records related to this Agreement for no less than three years after the <br />County Superintendent makes final payment or after termination of this contract and all <br />other pending matters are closed.
