Page 31 <br />Exhibit C Core Services Performance and Reporting <br />Contractor agrees to meet the following performance measures and provide the following data <br />and reports. Reporting is a requirement of payment and delays in submission of reports may <br />result in a delay in payments to Contractor. Payment is contingent upon complete, accurate, <br />and timely reporting. <br />A. Reports <br />1. Reports are due electronically to Chris Ratana (cratana@smcgov.org), or their <br />designee, in accordance with the table shown below. Contractor will provide: <br />a) A quarterly performance report that includes the following: <br />i. Results for all performance measures as described above; and <br />ii. A narrative on trends in services, needs, and performance, as well as <br />any changes to operations and/or funding sources; and <br />b) Emergency Financial Assistance (EFA) reports on a quarterly basis that <br />includes the following information: <br />i. Number of EFA requests received by city. <br />ii.
