Annual Action Plan <br /> <br />12 <br />OMB Control No: 2506-0117 (exp. 09/30/2021) <br />The Fair Oaks Community Center offers office space for local agencies to serve low to moderate <br />income residents in the community. This enhances coordination among agencies and creates a <br />“one-stop” shop for residents. <br /> <br />Private and Governmental Health – FOCC has partnered with San Mateo County Health to <br />provide a weekly site for the San Mateo County Mobile Clinic which provides convenient <br />and confidential drop-in medical care to adults 18 years and older. Additionally, the FOCC <br />staff administers the City’s Human Services Financial Assistance (HSFA) program which is <br />comprised of non-federal funding. HSFA provides monetary support to local non-profit <br />agencies whose programs respond to the human service needs of Redwood City residents. <br />Samaritan House, a nonprofit agency, was one of the recipients selected to receive HSFA <br />funding for PY25 for its Free Clinic of Redwood City, which provides primary and specialty <br />medical and dental services to uninsured, low-income residents. <br /> <br />Mental Health – As described above, the Fair Oaks Community Center offers a variety of <br />programs, creating easy access for low to moderate income households. This partnership <br />includes mental health providers and/or referrals to mental health providers. <br /> <br />Service Agencies – The City of Redwood City hosts a bi-weekly Homeless Outreach and <br />Provider’s Meeting. This meeting brings together various Redwood City homeless <br />providers, averaging about 20 people in attendance. The purpose of the meeting is to <br />coordinate efforts and to identify ways to collaboratively address homelessness in Redwood <br />City. <br /> <br />Describe coordination with the Continuum of Care and efforts to address the needs of <br />homeless persons (particularly chronically homeless individuals and families, families with <br />children, veterans, and unaccompanied youth) and persons at risk of homelessness. <br /> <br />The Continuum of Care (CoC) guides the implementation of the County’s housing and service <br />system to meet the needs of homeless individuals and families. As a member of the San Mateo <br />Continuum of Care (CoC) Steering Committee, the City aligns the CoC’s plans and strategies. The <br />City participates in CoC meetings and monitors CoC decision-making to better understand latest <br />trends in homeless outreach and service provision. And the City forms part of the CoC through <br />the City’s Fair Oaks Community Center which serves as the entry point for the Coordinated <br />Entry System (CES) for Redwood City. The City’s own Homelessness Initiatives build on and <br />leverage the countywide Continuum of Care system/Coordinated Entry System. <br /> <br />7.A. - Page 20 of 189 <br />224
