Annual Action Plan <br /> <br />13 <br />OMB Control No: 2506-0117 (exp. 09/30/2021) <br />Chronically Homeless: In 2022, the City launched a Coordinated, Inter-agency Homeless <br />Outreach Strategy Team (OS Team) comprised of 10 staff from partner agencies led by <br />LifeMoves with WeHOPE, Downtown Streets Team, Nation’s Finest, and Street Life Ministries. <br />This program ended 12/31/2024 and was transitioned into the County-funded Bayside <br />Homeless Outreach Team. This new program will continue to provide targeted outreach and <br />case management services to chronically homeless, unsheltered homeless residents living in <br />encampments, vehicles, and on the street in Redwood City. Staff on the Bayside Homeless <br />Outreach Team have a combination of lived experience of homelessness, speak Spanish, and <br />bring a diverse skillset that has shown to be successful in engaging and connecting the <br />unsheltered population in Redwood City to County services and additional housing supports <br />offered by the City. <br /> <br />Families with children: The City leverages the Countywide CES system to access services and <br />housing for families with children. In addition to family shelter sites in Redwood City, nearby <br />Menlo Park, San Mateo, and Daly City, the County system also includes an Offsite Shelter <br />Program offering additional shelter space at a motel in Burlingame. The CoC has a rapid re- <br />housing program (providing short-term rental assistance plus case management to homeless <br />families). <br /> <br />Veterans: The Continuum of Care continues to focus on ending Veterans homelessness. <br />Nation’s Finest is one organization with experience and access to resources available to address <br />homelessness for Veterans. While the City no longer directly funds Nation’s Finest since City’s <br />Homeless Outreach Strategy Team (OS Team) program ended in December 2024, Nation’s <br />Finest continues to be a partner countywide and in Redwood City. The City continues to <br />collaborate with them on issues related to Veterans homelessness. Nations Finest is also one of <br />the organizations that attend the City’s bi-weekly homeless outreach and providers meeting. <br /> <br />Youth: Currently, HUD CoC funds are provided to the San Mateo County Mental Health <br />Association (MHA) to operate its Support and Advocacy for Young Adults in Transition (SAYAT) <br />Program, which provides case management and housing search/stabilization services to <br />homeless youth, and the San Mateo County Housing Authority has a supply of 25 Family <br />Unification Program (FUP) vouchers for youth leaving the foster care system to ensure they do <br />not become homeless. <br /> <br />Addressing Persons at Risk of Homelessness: The City’s Fair Oaks Community Center provides <br />emergency financial assistance for persons at risk of homelessness. <br /> <br />7.A. - Page 21 of 189 <br />225
