<br /> Annual Action Plan <br /> <br />53 <br />OMB Control No: 2506-0117 (exp. 09/30/2021) <br />Planned Activities StarVista's Daybreak is a transitional housing program in Redwood City specifically designed to meet the <br />needs of homeless youth ages 16-21. It provides these vulnerable young people with safe and stable <br />housing, independent living skills training, and individualized case management and other <br />comprehensive services designed to help them make a successful transition from life on the street to <br />healthy, permanent living environments and self-sufficiency. By providing a safe and stable place to live, <br />healthy meals, and caring counselors and case managers, Daybreak helps vulnerable young people break <br />the cycle of homelessness, poverty, and addiction and get back on track with their lives and goals. <br />9 Project Name Climate Resilient Communities – Resilient Homes Minor Home Repair Program <br />Target Area CityWide <br />Goals Supported Special Needs Housing <br />Needs Addressed Creating Inclusive Communities w/Afford. Housing <br />Funding CDBG: $21,076 <br />Description Climate Resilient Communities provides extremely low, very low, low and fixed income residents who do <br />not typically qualify for Energy Savings Assistance and electrification benefits to receive energy and <br />water conservation solutions, as well as address de-carbonization, climate change, and household <br />safety. This project is eligible under 24 CFR 570.202 (a) (1) the CDBG regulations. <br />Target Date 6/30/2026 <br />Estimate the number <br />and type of families <br />that will benefit from <br />the proposed activities <br />The project will benefit 2 households. <br />Location Description 3921 Bayshore Rd, Palo Alto, CA 94303 <br />Planned Activities CRC provides home repair services to Extremely Low Income (ELI), Very Low Income (VLI) and Low <br />Income (LI) residents including those on a fixed income.
