<br /> Annual Action Plan <br /> <br />69 <br />OMB Control No: 2506-0117 (exp. 09/30/2021) <br />providers assess the needs that residents with serious mental illness face, monitor the <br />increasing rents, and support residents in accessing available services. Extensive housing <br />waitlists and insufficient income levels are expected to continue to affect a client’s ability to <br />rapidly transition to permanent housing. <br />Lastly, the City plans to continue its hotel respite program which bridges households from <br />unsheltered homelessness to shelter and services. The ultimate goal of this program is to <br />provide timely support to otherwise unsheltered residents (i.e. living on the streets, <br />encampments, or in vehicles) as they transition to permanent housing and other supportive <br />services (i.e. detox, sober living environment, skilled nursing facility, family shelter, etc.). <br />Helping homeless persons (especially chronically homeless individuals and families, families <br />with children, veterans and their families, and unaccompanied youth) make the transition to <br />permanent housing and independent living, including shortening the period of time that <br />individuals and families experience homelessness, facilitating access for homeless individuals <br />and families to affordable housing units, and preventing individuals and families who were <br />recently homeless from becoming homeless again <br />The City’s goal is to maximize access and effectiveness of existing state and federal housing <br />subsidies while also expanding the available housing subsidies. <br />Residents that are being discharged from public institutions, such as correction facility, foster <br />care, mental health care facilities or healthcare facilities, are at higher risk for homelessness. <br />The FOCC is one of eight Core Service agencies across the county offering supportive services, in <br />cooperation with the County Human Service Agency, to help individuals and families with much <br />needed support to stabilize their living situations. Many households at higher risk for <br />homelessness from these institutions are referred to the core service agencies, including the <br />FOCC, for additional support. <br />The FOCC also serves as one of the entry points for the San Mateo County Coordinated Entry <br />System (CES), providing initial screening and referrals for CES. Once referrals to CES have been <br />made, the CES Specialists complete the homeless services assessment providing either <br />diversion services or shelter. When shelter beds are not available, clients are placed on a <br />shelter waitlist until placement is available. The FOCC plays a critical role in helping connect <br />homeless individuals and families to shelter and other housing resources. FOCC also provides <br />financial assistance to help prevent homelessness and to support households transitioning from <br />shelter to permanent housing with deposit assistance. <br />The City will continue its housing subsidy program to both support and leverage existing state <br />7.A. - Page 77 of 189 <br />281
