Brief company overview including a description of the company <br />structure, and a statement on whether the company is a private or <br />public entity <br />c. Years in business. <br />d. Number of employees. <br />e. Annual sales volume. <br />f. Name and contact information (address, email, and phone number) of <br />Principals and/or Local Contacts. <br />2. Provide a list of any subcontractors, their business address, and a brief <br />summary of their role in the project. <br />3. Provide the name, location, and contact information of the nearest <br />authorized service center for maintenance and repairs. <br />4. Proven ability to provide installation support to others and service/support <br />after installation, meeting minimum requirements as specified herein. <br />5. Continuously worked with equipment manufacturer, including providing <br />installations and/or service, for minimum of three (3) years. <br />6. Approved in writing by manufacturer. <br />7. Documentation of manufacturer’s installation and/or service training within <br />previous two (2) years. <br />8. Similar system installed in three (3) or more customers of similar size and <br />complexity, at least one of which must be located within 4 hours driving time <br />from Owner’s location. Provide the following for each installation: <br />a. Name of project. <br />b. Location. <br />c. Contact name, telephone number and email address. <br />d. Date of installation. <br />e. Description of equipment and quantities. <br />H. Manufacturer’s Qualifications (if not the Contractor): Provide documentation: <br />1. In continuous operations for previous five (5) years. <br />2. Description of the manufacturer’s presence in the region. <br />3. Similar systems and system integration installed in three (3) or more <br />customers of similar size and complexity, at least one of which must be <br />located within 4 hours driving time from Owner’s location. Provide the <br />following for each installation: <br />a. Name of project. <br />b. Location. <br />c. Contact name, telephone number and email address. <br />d.
