Project Evaluation (Percent of Total Value: 15%) Within this section, <br />address the criteria that defines the Project Evaluation Rating Factor (see table below) <br />in a cohesive, comprehensive, and concise narrative format. <br /> <br />Project Evaluation: The applicant described how it will collect data and evaluate the <br />effectiveness of the project. The elements that comprise this Rating Factor are listed <br />below. Addressing each element does not in itself merit a high rating; although each <br />element is to be addressed (when applicable), it is the quality of the response to each <br />that will be evaluated. The response will be evaluated with a single rating based on a <br />scale of 0-5. <br />4.1 <br />Describe the plan to determine the staff and/or entity that will be conducting the <br />project evaluation and how monitoring activities will be incorporated into the <br />various phases of the project (e.g., start-up, implementation, service delivery <br />period). <br />• Provide the qualifications of the staff or evaluator selected to evaluate <br />the project, as they relate to experience with evaluation, data collection, <br />analysis. <br />• If an evaluator is to be selected after the grant is awarded, specify the <br />process and criteria for selection. <br />• Describe the preliminary plan for monitoring the project to ensure that <br />the project components are implemented as intended. <br />4.2 <br />Describe the plan to collect data to measure the success and impact of your <br />project in line with the goals and objectives listed in the Work Plan (see Rating <br />Factor 2.2). If the outcomes you propose to track require you to collect data <br />from an outside entity, describe your plan for obtaining data and entering into <br />data sharing agreements if needed. <br /> <br /> <br />7.C. - Page 42 of 101 <br />101
