It is strongly recommended that relocation applications are submitted <br />through the online portal to ensure the relocation application is <br />submitted on time. <br />d. If an incomplete relocation application is submitted before the deadline, to <br />the extent possible, City staff will notify the applicant that the relocation <br />application is incomplete and that the tenant may submit missing <br />information/documentation by the deadline in order to be considered <br />complete. Any missing information/documentation received after the <br />deadline will not be considered. <br />e. If a relocation application is received after the deadline, it will be considered <br />invalid and ineligible for consideration for greater relocation assistance <br />benefits. <br />f. Tenants are encouraged to submit a relocation application earlier than the <br />deadline to ensure the City receives a complete application. <br />3. Requirement to provide proof of income: Tenants will be required to provide proof of <br />household income to determine if they qualify as an Eligible Residential Household. <br />The following documents will be accepted to prove household income. Proof of <br />income must be submitted for all household members who are 18 or older. <br />Common Sources of Income <br /> <br />Sources of income: <br />Examples of verification documentation to include with <br />the application <br />(Only one of these is required per source of income): <br />Hourly wages or salary form <br />employment. <br />• Includes: Tips, commission, <br />overtime pay, bonuses, etc. <br /> <br />• Most recent year’s complete income tax return <br />• Most recent months’ pay stub <br />• Most recent IRS W2 form(s) <br />• Bank statement showing deposits of income for the <br />most recent month <br />• A document from an employer attesting to the <br />employee’s income <br />Self-employment income as an <br />independent contractor, etc. <br />• Most recent year’s complete income tax return <br />• Most recent months’ pay stub <br />• Most recent IRS W2 form(s). <br />• Bank statement showing deposits of income for the <br />most recent month <br />• A list of clients, average monthly earnings per client, and <br />clients contact information <br />8.A. - Page 40 of 85 <br />298
